During my foundation year of the MSW program I provided individual counseling services to a 23-year-old female patient at Kaiser Permanente. This case summary analysis and reflection paper is based on the sessions I provided to this patient. During the intake session, the patient reported she was admitted into the emergency room for a total of three times in the last 3 months due to persistent panic and anxiety symptoms. The patient indicated that she began to excessively worry about "the possibility of dying and becoming ill" and that her increase in symptoms were was triggered by her role as a caregiver. She indicated that her mother is diagnosed with Multiple Sclerosis and her grandmother has both Chronic Obstructive Pulmonary Disease and Cystic Fibrosis. The patient also shared how she and her family were struggling financially and wanted to contribute to the family’s expenses.
The patient reported that her overwhelming fear was not triggered by any recent significant event and her current functioning was surprising to her. Moreover, the patient expressed that in addition to her fear of dying, fear regarding her general safety also became heightened such as being followed or having a car accident. Her symptoms were too overwhelming to handle and hoped that counseling would help to acquire coping skills to manage her symptoms. During the intake, it was found that patient had a supportive husband of 7 years and three young children, all under the age of 5.
