Advantages And Disadvantages Of Patellar

There is a controversy over ACL reconstruction techniques. On the one hand, doctors argue that the patellar tendon autograft is the best technique. On the other hand, they argue that the hamstring autograft is the best technique. I believe that the best technique depends on the patient and their activity levels. People who compete in sports at a higher level than high school or college for four years would benefit more from a patellar tendon graft. People who do not participate competitively in sports for a long period of time are likely to benefit more from a hamstring graft. Making the right choice in reconstruction techniques is important to patients and their rehabilitation.

The patellar tendon graft is the strongest graft initially after surgery (Lowe). While this is scientific fact, the hamstring graft has proven similar stability based on the Lachman test (Bhandari, Farrokhyar, and Poolman). Doctors Bhandari, Farrokhyar, and Poolman state, “sensitivity analysis focusing on state of the art hamstring graft fixation techniques further weakens the evidence that bone patellar tendon-bone autographs provide better stability.” By providing this information from a study, it is shown that there is not a significant difference between the stability of the two types of autografts. A smaller number of future revisions have been found in patients who received the patellar tendon graft than patients who received the hamstring graft. However, the statistics are not overwhelmingly different. Patellar tendon grafts result in a 2.8% revision, while hamstring grafts result in a 4.2% revision rate (Geier). Many will argue that the patellar tendon graft is stronger, but the strength difference between the grafts is not …show more content…
Relative to this issue, the hamstring graft has been proven to have the least amount of post-operative pain (Lowe). The pain related to the patellar tendon graft is an issue because it often leads to and increased chance for patellar tendonitis and patella fracture (Lowe). Due to the increased pain, the patellar tendon graft causes the initial rehabilitation to be more difficult. The hamstring graft provides the patient with an easier start to rehabilitation, which is extremely important to the healing process. An easier start to rehabilitation provides the patient with a better attitude to continue and push through the entire rehabilitation process, which is crucial to recovery. Difficult initial rehabilitation can create a negative attitude and cause patients to quit

Thirty two patients had sustained an UCL avulsion fracture and nineteen patients sustained an UCL rupture. For the patients with an UCL rupture the surgical technique used consisted of, the surgeon drilled into the proximal and distal insertion sites for the UCL ligament using a 2.5 mm bit to construct the tunnel for the free tendon graft, for sixteen patients the palmaris longus tendon was used as the graft and for three patients the split flexor carpi radialis tendon was used, that was used in the procedure. The surgeon then used passing sutures to put both ends of the graft through the tunnel with about 1 cm of the graft in each tunnel. To stabilize the free graft tendon endings in each tunnel the surgeon placed 3 mm bioabsorbable suture anchors in the tunnels and placed them through the graft and tied them off.…

The hamstring consists of a group of three muscles; the Biceps Femoris, the Semitendinosus, and the Semimembranosus all of which run along the back of the leg just above the knee. These muscles are vital to the proper function of the knee and leg and can affect a person's ability to walk, run, kick, and perform other physical activities at full strength. One of the most important jobs of the hamstring is knee flexion, but the hamstring also helps the upper body distribute its weight evenly over the lower body so people don't fall or stumble when they try to walk or run. Hamstring injuries are rated on a scale from one to three.…

The pain began slowly, pricking me with its sharp needles only while doing barre in ballet once a week. I ignored it, believing it to be normal, common discomfort that would soon go away, typical thoughts of a dancer whose entire sport is centered around “good pain”. Six months later, it had escalated to the degree that every step I took felt like an arrow to my knee. Dancing had become impossible, and it was determined that I should be taken to the orthopedist. The bland, brown and beige lobby became extraordinarily familiar as I waited two hours to be examined.…

The ACL is the center of much responsibility when it comes to the knee. The ACL runs across the knee joint, and keeps the shin from sliding forward, and provides stability when performing rotational movements. It is one of the many ligaments that holds the knee joint in place, and keeps the knee from buckling.…

Later that night I could feel them wearing off, but there was no pain, until the first day of rehab. I wasn’t sure what to expect the first day of rehab, I’ve never had an injury this devastating.…
