Pulseless V-Tach (Torsades de Pointes): slow IV Push over 6 to 10 minutes

V-Tach (Torsades de pointes) with a pulse: IVPB drip over 6 to 10 minutes.

Refractory bronchospasm: IVPB drip over 6 to 10 minutes.

Dose
Adult: CPR (pulseless Torsades de pointes): 2 Grams of a 10 percent solution infused IV push over 6 to 10 minutes.

V-Tach (Torsades de pointes with a pulse) 2 grams in 250 mL NS infused IVPB over 6 to 10 minutes.

Refractory bronchospasm: 2 grams in 250 mL NS infused IVPB over 6 to 10 minutes.

Pediatric dose: Not used in prehospital setting.

Drug Action:
Reverses magnesium deficiency
Physiologic calcium channel blocker - relaxes smooth muscle
Central nervous system depressant.
Magnesium sulfate

Indications:
Refractory V-Vib and V-Tach due to magnesium deficiency (alcoholics, malnutrition, anorexia, prolonged diarrhea, etc).

Refractory bronchospasm in asthma

V-Tach that presents as Torsades de pointes configuration

Seizures secondary to eclampsia

Precautions:
Inform BioTel when magnesium deficiency is suspected.
Continually monitor blood pressure, respirations, ECG, LOC, and muscle strength.
Inform Biotel when patients are on Digitalis preparations.
If only available in a 50 percent solution, DILUTE 2 grams (4mL) of a 50 percent solution with 16 mL of NS - 10 percent solution.
Magnesium sulfate

Side Effects:
When given overdose or too rapidly, its physiologic calciumchannel blocking properties interfere with calcium uptake into muscle roups. The following dangerous side effects may then occur:

Muscle weakness/paralysis, Respiratory depression, CNS depression, Hypotension, shock, Bradycardia.

Shock or systolic BP less than 110 mmHg
Respiratory Depression
Heart block
Chronic renal disease/dialysis
Magnesium sulfate
