Pediatric: Not routinely used in the prehospital setting.

Drug Action
By blocking the movement of calcium into muscle, diltiazem relaxes vascular smooth muscle, causing vasodilation.

Slows conduction through the AV node.

Decreases the rate of ventricular response in supraventricular tachycardias.

Exerts negative chronotropic and inotropic effects - decreases myocardial O2 demand.

Indications
Rapid venticular rates associated with atrial fibrillation and atrial flutter PSVT unresponsive to Adenosine.

Precautions
Continually monitor blood pressure closely
Incompatible with Lasix; flush IV line well between drugs.
Inform BioTel if patients are on Digitalis preparations of beta-blockers.

Hypotension
Wide complex Tachycardias (can cause v-tack to deteriorate into v-fib).
History of Wolff-Parkinson-White Syndrome (WPW).
2nd and 3rd degree heart block
