-gather objective and subjective data
-review knowledge of the client's condition/Tx -use previous experience -apply standards to determine the expected outcome of care |
Critical thinking questions to ask yourself:
Has the client's condition improved?
Is client able to improve? -are there physical factors preventing recovery To what degree does the client's motivation/willingness influence their response to Tx? |
How long do you have to evaluate for?

What are the 5 elements of the evaluation process?

1-identifying evaluative criteria/standards (know what to look for and set goals to meet expected outcomes of care)
2-collect data to determine whether criteria/standards are met (helps to determine client satisfaction as well as strengthen partnership btw you and client) 3-interpret/summarize findings (compare behavioral/physiological S+S with what is expected) 4-document (allows you to see progress) 5-terminate/continue/revise care plan (ask if goals have been met) |
How do you know that the expected outcome has been met?
a) First examine the expected outcome to identify the desired behavior/response
b) Then assess the client's actual behavior/response c) Compare these d) Judge the degree of agreement between the two and if they are in disagreement--find out why? |
The evaluation process is easier to perform after caring for a client over a ________ period of time.
long
