The purpose of this paper is to address how the failure originated and the results and impact of the failure on the Terrebonne Parish Juvenile Detention Center came about. Also, how the leadership (or lack thereof) directly impacted the outcome of the event. By identifying and discussing the results of the investigated topic this paper should also discuss how the failure might have been prevented, steps taken for improved conditions, and solutions for preventing failures in the future.

According to the text, a normal accident is defined as when complex technological elements malfunction and human operators misjudge what’s happening, responding with actions that can accelerate the deterioration of the situation. With this definition, its supports that in this situation normal accident applies because it shows how initial misperception’s in the rapidly arising situations within the Juvenile Detention Center had set a tone for further misreads fueling incorrect responses from management and supervisors that led to the failure of the Detention Center, while the structure of the administration fail to realize the downfall of the youths due to the harm that was brought upon them while in custody.
The Detention Center’s administration, employees, and staff may have found themselves in unfamiliar water, or unplanned and unexpected circumstances that may not have been immediately comprehensible, but which also demand immediate decisions from stressed personnel.
