Internship licensure requirements

a. 50 Hours College Credit with average not less than C
b. Currently enrolled in and in good standing in an accredited school or college of pharmacy
c. Intern certificate becomes void 5 years after issuance
Licensure exam requirements
1500 pro-practice hours
Non-college hours
must be reported by preceptor to state board every 240 hours
Intern license display requirements
a. Must be conspicuously displayed
b. An intern MUST ABIDE by the regulations governing same, whether logging hours for credit of not.
c. Multiple locations-may request duplicate certificate-fill out form on state board website listed above.
Intern practice requirements
a. Must be in a licensed training area
b. Under direct supervision of a licensed preceptor
c. One intern to one preceptor
d. Professional Conduct a must
e. Address and job changes (non college practice only) 10 days notice
f. Cannot supervise a tech. Can do all other pharmacist duties. Cannot serve as a technician or clerk.
g. Must wear intern badge
h. Must identify self as intern on all incoming and outgoing phone calls.
i. Can't work more than 40 hours (won't be counted)

k. You must make a request in writing to have your intern hours transferred to another Board of Pharmacy.
DEA numbers
a. DEA number consists of two letters and 7 numbers
b. Add 1st, 3rd and 5th numbers together
c. Add 2nd, 4th and 6th numbers together and multiply by 2
d. Add B to C and last digit of this sum will be the same digit of the 7th number if the DEA # is valid
