Inflammation

chronic inflammation may eventually lead to disease/conditions (cancer, rheumatoid arthritis)


Infl. response mediated by histamine, serotonin, bradykinin, leukotrienes, and prostaglandins


Last two have a major influence on inflammation symptoms

Gout

caused by overproduction of uric acid or decreased uric acid secretion (or both). Too much uric acid cause uric acid crystals to deposit in joints/tissues causing inflammation and extreme pain

NSAIDs

antipyretic, anti-inflammatory, analgesic properties


ASA- inhibit platelet aggregation (MI protection)


MOA: inhibit the 2 pathways


NSAIDs block COX enzyme


ASA- only NSAID that has antiplatelet effects



Indications: antipyretic, antiinflammatory, analgesic (give tylenol ONLY to children, do NOT give ASA)


ASA- platelet aggregation prevention (MI, stroke)


Arthritis- rheumatoid, osteoarthritis



CI: do NOT use in children w/ viral illness/ flulike symptoms-> Reye's syndrome (life-threatening, neurological deficits), VK deficiency/peptic ulcer disease, pregnancy 3rd trimester, increases bleeding potential



AE: GI- mild heartburn to severe GI bleeding (N&V). Acute renal failure- common in dehydrated patients, poor renal fx, taking diuretics.



Interactions- anticoagulants, corticosteroids, diuretics

Misoprostol

(Cytotec), given w/ NSAIDs to help prevent GI bleeding/ulcers

NSAID OD Management

adults- see tinnitus, hearing loss


children- hyperventilation, CNS effects (dizziness, drowsiness, behavioral changes


Given activated charcoal

Aspirin

ASA, salicylate, daily use 81mg or 325 mg, for prophylaxis of stroke, MI risks in adults; prevention of thrombotic events

Indomethacin

(Indocin), used for arthritis, promote closure of ductus arteriosus and used for preterm labor


Acetic Acid Derivative

Ketorlac

Toradol, powerful analgesic effects (used post-op for moderate to severe acute pain control via IV push). Think of this like a "big" Motrin. Can use for up to 5 days

Ibuprofen

(Motrin, Advil): prototype NSAID. Analgesic, arthritis, antipyretic


Proponis acid derivative

Antigout Drugs

Used to decrease symptoms of acute gout attack, prevent future gout attacks


Reduces uric acid levels in body


MOA: reduce production of uric acid by inhibiting enzyme called xanthine oxidase, responsible for converting purines into uric acid

Allopurinol

(Zyloprim): inhibits xanthine oxidase. For pts with excessive uric acid production.
