Trendelengburg test
- Demonstrates weak hip abductors such, L5 radiculopathies, superior gluteal nerve lesion
- Pt stand on one leg - Abnormal = opposite hip drops |
Ober test
- Demonstrates tight IT band or variety of neurological disorders such as polio and meningomyelocele
- Patient lays on side, knee flexed, hip in slight extension (allows IT band to pass behind greater trochanter) - Support knee and ankle - Drop knee, hip should adduct beyond anatomic neutral |
Thomas test
- Demonstrates tight ilipsoas, kidney stones, pancreatitis
- Patient flexes both knees to flatten lordotic curve - Should be able to fully extend each leg |
Patrick test
- Figure 4 - hip flexed, abducted, externally rotated and extended
- Gently press knee while stabilizing ASIS - Pain indicates hip arthritis or SI joint pathology |
Hip compression/ scour test
Same as Patrick but force in compressive through femur to hip joint
Hip drop test
- Tests ability of lumbar spine to sidebend
- Contact iliac crest posteriorly - Patient bends one knee - Convexity should be on side of bent knee and iliac crest should drop an inch or so |
Anterior Trochanter CS
- Sartorius
- Medial and 1-2 in. inferior to ASIS - Supine, flex, abduct, externally rotate |
Posteromedial trochanter
- Gemelli and quadratus femoris
- 4 cm below trochanter on posteriormedial surface of femur - Indication - ischiogluteal bursitis - Pt prone, leg extended, adducted, knee flexed externally rotated - Physician stands on opposite side, grasp knee, hold leg between arm and torso |
Posterolateral trochanter
- Posterolateral superior surface of trochanter
- Indication - trochanteric bursitis - Pt prone, hip extended, knee extended, abducted, externally rotated, brace with knee |
Lateral trochanter
- Lateral surface of greater trochanter, 12-13 cm distal
- Trochanteric bursitis or IT band dysfunction - Pt prone, abducted and externally rotated |
Gluteus medius
- 1 cm below iliac crest in an arc 3-4 cm long
- Pt prone, leg extended and internally rotated |
Piriformis
- Belly of piriformis muscle
- Pt. prone, flexed, abducted, externally rotated - Physician seated below table |
