Paget disease of breast

Skin: pruritic, red, scaly located in the nipple spreading towards areola. Looks like dermatitis, psoriasis or eczema.

Vaginal discharges smells

Foul smelling: bacterial vaginosis (Gardenella Vaginalis) and Trichomoniasis (T. Vaginalis)


Non foul smelling : Chlamydia, Gonorrhea and Candidiasis


Vaginal infections

Bacterial Vaginalis:


Grey white discharges


Fishy smell


Clue cells


Vaginal pH > 4.5



Management:


Metronidazol oral and clindamycin cream



Trichomonas Vaginalis:


Greenish, Frothy with vulvovaginitis


Strawberry cervix. Foul smelling.



Candidiasis:


Cheesy discharge. Non foul smelling




Stages of labour

Stage 1: onset of contractions to fully dilated cervical os


Latent phase: onset of contractions to beginning to dilate.


Active phase: 3-4 cm dilatation to full dilatation



Stage 2: Full dilatation to delivery of fetus



Stage 3: delivery of fetus to delivery of placenta

Investigation:


Women under 40 years having two raised level of FSH (more than 40 IU/l) at least 4 weeks apart



Management:


HRT till menopausal age (51)

Cervical screening test

1st: Cervical smear


If abnormal / mild dyskaryosis


2nd: HPV testing


3rd: Colposcopy


If abnormality beyond cervical canal


4th: Cone biopsy


5th: LLETZ

Pregnant in pain

Always prescribe only paracetamol...

Eclampsia

C/f:


Pre eclampsia + tonic clonic seizure



Management:


Control:


Loading: 4g MgSO4 in 100 ml 0.9% NS over 5-10 mins


Maintenance: 1g/hr maintained for 24 hrs



Recurrent seizure:


2 g MgSO4 bolus Or


1.5-2 g/hr

Contraception

1st line for young women: OCP, POP, implants (Nexplanon)



1st line for above 20 yrs: IUS (Mirena)
