Hypnosis

-American Society of Clinical Hypnosis is only organization that formalized standards of training and certification process

-Research done in L&D, Derm, peri-menopausal symptoms, dental pain

-Six classic stages: intro, induction, deepening, therapeutic suggestions, awakening and debriefing.

GUIDED IMAGERY

-Indications: people facing medical procedures, mental health conditions, smoking cessation, weight loss, recovery of subsatnce abuse, cardiac/hypertension/diabetic patients, insomnia, dementia, asthma, copd, functional pain disorders, osteoarthritis, stroke.

-Possible contraindications: paranoid schizophrenia, anxious people in whom the exercise increases anxiety, trauma survivors.

-Artists are more likely to benefit quicker, adolescents respond better.

AUTOGENIC TRAINING

-The practitioner repeats a set of statments and visualizations that induce a state of relaxation. The practice influences autonomic nervous system to balance sympathetic and parasympathetic.

-My arms are heavy, my arms are warm, my heartbeat is calm and strong, my breathing is calm and relaxed, my abdomen radiates warmth, my forehead is pleasantly cool.

-Try in sleep, fibromyalgia, anxiety, IBS, headache.

BIOFEEDBACK

-Four most common forms of biofeedback are heart rate variability (changes in heart rhythm), EMG, EEG, thermal feedback (which measures skin temp).

-Primarily used to treat stress and stress related conditions such as high blood pressuronic pain, re, tension headache, migraine, anxiety, depression, insomnia, IBD, chronic pain, concentration difficulties.

-Heart Math uses heart rate variability. You adjust heart rate variability by adju.sting breathing rate. Improves regulation of emotionally mediated reflexes throughout the body

-Neurofeedback (EMG). Helpful in modifying seizure activity, attention, traumatic brain injury, chronic pain, autistic behaviors, headache, migraine, depression, anxiety, addiction, sleep problems. Useful in learning disability. Can help performance of athletes, artists, executives.

-In kids biofeedback has evidence for headaches, dysfunctional voiding, chronic abdominal pain, ADHD, asthma.

MBSR (Mindfulness Based Stress Reduction)

-Deliberate, sustained , non-judgemental way of paying attention to one's experience that is said to enhance self-awareness, change maladpative thinking, and increase awareness.

-Body scan, mindful movement.

-One mechanism is helping patient recognize primary sensory experience (like pain) compared to secondary experience (emotional or cognitive process created in reaction to primary experience).

MINDFULNESS BASED COGNITIVE THERAPY

-Used for depression, borderline personality, eating disorder, childbirth, substance abuse.

PROGRESSIVE MUSCLE RELAXATION

-Create tension for 10 seconds, then let muscle relax. No certification needed.

BREATHING TECHNIQUE

-The most effective way of relaxation.

YOGA

-Postures are called asanas.

-Pranayama is breath control.

-Dharan is concentration.

-Dyana is meditation.

-Yoga down regulates the HPA axis and sympathetic nervous system. It decreases salivary cortisol, blood sugar, plasma renin, 24 hour urine catecholemines, CRP, and cytokines.

-Can lead to slower respiration, decrease in resting heart rate, enhanced heart rate variability, enhanced baroreflex sensitivity, and decrease blood pressure.

-Inner city 4th and 5th graders had fewer negative behaviors in response to stress after 2 weeks of yoga.

-Helpful in eating disorders, treatment of OCD, phobias, dyslexia, grief, PTSD, chronic pain, and insomnia.

TAI CHI

- Research done in coronary disease, metabolic syndrome, back pain, chronic fatigue, stroke rehab, pain, osteoporosis.

STRESS RESPONSE

-Non-essential systems inhibited like digestion, growth, reproduction.

-Amygdala stimulation has role in generation of fear and anger.

-Catcholamines stimulate gluconeogenesis and hepatic glucose secretion, which can increase risk of metabolic syndrome.

-Sickness syndrome refers to sympathetic neuron stimulation and IL-6 releast causing degradation of mast cells and inflammatory response.

-Two steps required in relaxation response: Repetition of word/thought, and Passive return when other thoughts intrude.

HYPNOSIS & IMAGERY

-Six classic stages. Introduction, induction, deepening, therapeutic suggestions, awakening, debriefing.

-Wealth of research for use in kids.

-Use for pain, functional abdominal pain, anxiety, asthma, habit disorders, chemo induced nauseas and vomiting, insomnia, hypertension, anxiety, preparation for surgeryh

MUSIC THERAPY

-Has a positive effect on neuro-hormonal and immune systems and its links to pain perception and emotional processing.

-Reduces pain response and anxiety in variety of pediatric settings.

-Positive studies on music therapy and autism.

MIND-BODY THERAPY IN PEDIATRICS

Some of the most common uses: recurrent abdominal pain, post chemo neuropathy, headaches, enuresis, encopresis, anxiety, phobias, IBS, fibromyalgia, ADHD.

MOTIVATIONAL INTERVIEWINGFive Steps:1. Empathy2. Develop discrepancy3. Avoid argument4. Roll with patient resistance5. Support self efficacy
