Drug Name: EPINEPHRINE 1:10,000 (adrenalin)
Dose: Cardiac arrest IV/IO 1mg(10mL of 1:10,000) every 3-5min flush with 20mL saline. PEDS IV/IO 0.01 mg/kg(0.1mL/kg) of 1:10,000 solution every 3-5 min Side effects: Hypertension, tachycardia, arrhythmias, pulmonary edema, anxiety, restlessness, psychomotor agition, nausea, headache, angina |
Indications: Cardiac arrest (v-fib/pulseless V-tach, asystole, PEA), symptomatic bradycardia as an alternative infusion to dopamine, severe hypotension secondary to bradycarida when atropine and transcutaneous pacing are unsuccessful, allergic reactions, anaphylaxis, asthma.
Contraindication: hypertension, hypothermia, pulmonary edema, myocardial ischemia, hypovolemic Mechanism of Action:Direct-acting alpha- and beta-agonist. Alpha: vasoconstriction. Beta-1: positive inotropic, chronotropic, and dromotropic effect. Beta-2: bronchial smooth muscle relaxation and dilation of skeletal vasculature |

Drug Name: ADENOSINE (Adenocard)
Dose: ADULT- 6mg rapid IV, flush with 20cc of normal saline. elevate the arm. If no response in 1-2 min may repeat 12mg, up to twice. PEDS0.1mg/kg rapid IV, flush with 2-3mL of normal saline. If no response in 1-2 min dose can be doubled 1 time (0.2mg/kg) not to exceed 12mg Routes to be given: Rapid IV push Side effects: Facial flushing, shortness of breath, chest pain, headache, paresthesia, diaphoresis, palpitations, hypotension, nausea, metallic taste |
Indications: Conversion of supraventricular tachycardias with no known atrial fibrillation or atrial flutter.
Wide complex tachycardia of uncertain origin unresponsive to lidocaine. Contraindication: Sick sinus syndrome, 2nd or 3rd degree AV blocks; except in patients with a functioning ventricular pacemaker. Use cautiously in patients with known asthma (has precipitated acute bronchospasm). Patients on theophylline and related methylxanthines. Patients on dipyridamole (Persantine) or carbamazepine (Tegretol). Cardiac transplant patients are more sensitive to adenosine and require only a small dose (relative). Known atrial fibrillation or atrial flutter. Pregnancy (no controlled studies) Mechanism of Action:Slows conduction time through AV node; can interrupt re-entrant pathways through the AV node. Slows sinus rate. Larger doses decrease BP by decreasing peripheral resistance. |

Indications: Rapid ventricular rates associated with atrial fibrillation and atrial flutter, and for PSVT refractory to adenosine.
Contraindication: Hypotension (less than 90 mmHg systolic), Acute Myocardial infarction, Cardiogenic shock Ventricular tachycardia or wide-complex VT of unknown origin, Second or third-degree AV block Wolff-Parkinson-White (WPW) syndrome, Sick Sinus syndrome, Beta Blocker Use Mechanism of Action: Slows the rapid ventricular rate associated with atrial fibrillation and atrial flutter, and reduces coronary and peripheral vascular resistance. |

Dose: ADULTS-2.5 - 5.0 mg IV PUSH over 2-3 minutes.
