Imipenem is compounded with cilastatin to protect it from metabolism by renal dehydropeptidase

Imipenem resists hydrolysis by most beta lactamases but not the metallo -beta lactamases.

They are useful in empiric therapy cos they are active against beta lactamase producing gram positive and gram negative organisms, anaerobes and P. aeruginosa.

Meropenem and doripenem have similar activity with imipenem

Distribution : Imipenem/cilastatin and meropenem penetrate well into the tissues and fluids including csf when the meninges are inflamed.


Meropenem reach therapeutic levels in the bacterial meningitis even without inflammation


Elimination : by Glomerular filtration

Adverse effects : Imipenem /cilastatin can cause nausea
