Simple Bone Cyst (hemorrhagic bone cyst, idiopathic bone cavity)

Radiographic appearance?
* Well-circumscribed radiolucent area
* From 1-10 cm in diameter
* May scallop up between roots, a highly suggestive but not diagnostic feature
* Teeth generally vital, without root resorption
* Rarely, may be multilocular, with expansion
Aneurysmal bone cyst

Radiographic appearance?
* Unilocular or multilocular
* May be well-defined or diffuse
* Significant cortical thinning and displacement
Focal osteoporotic marrow defect

Radiographic appearance?
* RL or with fine trabeculation
* May appear well- or poorly-defined
Paget disease of bone (Osteitis Deformans)

Radiographic appearance?
* Early on, see decreased radiopacity
* Altered trabecular pattern
* In skull, may see large, circumscribed, radiolucent areas
* Later, get patchy sclerosis, or "cotton wool" appearance
* Teeth may show hypercementosis
* must rule out cemento-osseous dysplasia
Central Giant Cell Granuloma

Radiographic appearance?
* Well-demarcated, non-corticated RL lesions
* Unilocular (most cases) or multilocular
* Can cause expansion or perforation of cortical plate
Cemento-osseous dysplasia
(Focal)

Radiographic appearance?
*Most are mixed RL/RO with a thin RL rim
* Usually well-defined, but with irregular borders
Cemento-osseous dysplasia
(Periapical)

Radiographic appearance?
* Usually multiple foci
* May appear as periapical RLs
* With time, multiple lesions fuse
* Serial radiographs show progressive opacification
* Final result is opaque mass surrounded by a thin RL rim
* Periodontal ligament is intact
* Does not expand cortex
Cemento-osseous dysplasia
(Florid)

Radiographic appearance?
* Multifocal, often symmetrical
* Same maturation pattern as the other types
* May present with sequestra
Cementoblastoma

Radiographic appearance?
* Radiopaque mass fused to one or more roots, thin radiolucent line surrounding mass
* PDL may be hard to see
* May see root resorption
Osteosarcoma

Radiographic appearance?
* RL, RO, or mixed
* Margins are poorly defined
* "Sun-ray" appearance: bony trabeculae radiating from periphery of periosteum. Present in only 25% of cases and not unique to osteosarcoma
* Early feature: localized symmetrical widening of PDL space (Garrington's sign)
