Beta Lactum Antibiotics

Penicillin, cephalosporin, carbapenems, monobactams.




-Cross allergic reactions with all beta lactic meds. Esp. anaphylactic, angioedema reactions.

Sulfonamides

-Broad spectrum


-High concentration in kidney


-Tx for UTI's, pneumonia and respiratory infection

Sulfonamides:


Sulfonmethozole + trimethoprin (not sulfa)

Are used to treat UTI in women with short urethras after intercourse.

Adverse effects of sulfonamide:

-Photosensitivity (avoid light)


-Aplastic anemia, hemolotic anemia, thromnocytopenia (monitor CBC data)


-LIVER


-Need to monitor clients with diabetes because sulfonamide can cause low blood sugar



Drug interactions of sulfonamides:

Phenytoin toxicity


W/ warfarin can cause bleeding/hemorrhage


May cause pregnancy when used with contraceptives

Patient teaching of sulfonamides:

-take with a full glass of water 2,000-3,000


-Assess patient prior, during and post


-Monitor blood sugar of clients with diabetes


-watch for superinfection


-take with food


-Avoid direct sunlight and tanning


-Take full term as prescribed


-Teach client to know s/s of adverse effects and drug contraindication

Penicillins

-trade names do not always end in -cilin


-patients allergic to penicillin have an increased risk of allergy to other beta lactums. Most common reaction: rash and itching


-IV penicilin has the most common allergic reactions


-IM pcn, hold patient 20-30 minutes after injection to ensure there is no anaphylactic reaction when pt. gets home


-NSaids may be beneficial with pcn

Drug interactions of penicillin:

-warfarin


-contraceptives


-avoid taking with citrus juice


-if diarrhea, take probiotic


-superinfection common, candida

How to administer Penicillin:

-Do not give deep IM


-Do not mix IV


-Give 1 hour before meal or one hour after meal


-take at same time every day

Macroglide


(ex: erythomycin)

-used for strep


-IS GIVEN TO PT. IF ALLERGIC TO PENICILLIN


-common adverse reaction: N/V, abdominal pain


-hepatotoxicity, need to look at other meds with kidney or liver hepatotoxicity


-hearing loss or tinnitus


-affects contraceptive effectiveness


-CAN CAUSE LIFE THREATENING DYSRHTYMIAS. ASSESS HEART AND HEART MEDICATIONS


-some micro glides not recommended with orthostatic drugs


-not given with fruit juices


-full glass of water
