Measure and record radial pulse

1. Explain procedure.
2. place both index finger and middle finger on the thumbs side on the wrist to find the radial pulse
3. Count for one full minute
4. Call light is in reach
5. Before recording, wash hands
6. Radial pulse is within plus or minus 4 beats of evaluator's reading

1. Explain procedure
2. Provide privacy
3. Remove dirty gown and place it in the dirty container
4.grab a basin and add warm water and make sure you and the resident check the water.
5. Put on gloves
6. Start with the eyes. wash eyes with wet cloth. go from inward to outward and then continue to face
7. dry face with clean towel.
8. place towel under exposed arm
9. get a new wash cloth and apply soap
10. wash arm, hands, and under arm.
11. rinse and dry arm, hands, and under arms.
12. Move the body gentle
13. put on new clean gown on resident
14. empty, rinse, and dry basin.
15. after doing step 14, place it in a dirty designated supply area. (ddsa)
16. throw linen in a linen container.
17. Avoid contact with resident when adding a linen
18. after placing basin in DDSA, remove and dispose gloves into waste container and then wash hands.
19. call light is in reach and bed is in lowest position

Ted Hose

1. Explain procedure
2. Provide privacy
3. client is in supine position and while the stocking is being placed.
4. Turn ted hose inside out
5. place foot of stocking on top of toes and heel
6. Pull stocking over toes and heel and leg
7. Move foot and leg gentle
8. finish with no wrinkles, heel is in place, an opening in toe area
9. call light is in reach and bed is lowest position
10. after skill is done, wash hands

Ambulate with gait belt

1. explain procedure, speak clearly and slowly.
2. Before assisting,candadate ensures client is wearing shoes.
3. before assisting to stand, bed is at a safe level.
4.before assisting to stand, checks +/or locks bed wheels.
5. before assisting to stand, client is assisted to sitting position with feet flat on the floor.
6.before assisting to stand, applies transfer belt securely at the waist over clothing/gown
7. Before assisting to stand, provides instructions to enable client to assist in standing including prearranged signal to alert client to begin standing.
8. stand facing client positioning self to ensure safety of candadite and client during transfer. counts to three (or says other prearranged signal)
9. Assist resident to stand by grabbing transfer belt on both sides.
10. walk behind and aside within 10 ft. still holding onto the belt.
11. take resident to bed and remove gait belt.
12. call light is in reach and bed is in lowest position.
13. When done, wash hands
