Asthma step ladder management

Step 1: Inhaled SABA


Step 2: Inhaled steroid (200 -800)


Step 3 & 4: Inhaled LABA (3) > increase inhaled steroid to 800 to max (4)> Leukotriene receptor antagonists or theophylline SR.


Step 5: Steroid Tablets

Tension Pneumothorax

Presentation: acute respiratory distress, *raised JVP*, hypotension, +/- tracheal deviation



Management:


1. High flow O2.


2. Insert large bore cannula to 2nd intercostal space in the midclavicular line. Hissing sound is diagnostic


3. Chest drain in mid axillary line

Mesothelioma

Tumor of Pleura due to occupational exposure of asbestos



Presentation: Chest pain, Shortness of breath, weight loss +/- Metastasis (lymhadenopathy, hepatomegaly, bone pain, abd pain)



Inv: CXR or CT scan reveals pleural thickening or effusion. Dx by histology of pleural fluid, pleural biopsy and pleurodesis. Usually post mortem



Rx: Surgery and radiotherapy



Note: consult Coroner in case of "unnatural death" and he will give the final verdict and issue death certificate

Sarcoidosis

Lofgren syndrome



CXR: 1. Bilateral hilar lymphadenopathy


2. Erythema Nodosum: Lesion on the shin


3. Arthritis
