What is acceptable risk? |
In a calculated way, - We will take risks to save saveable lives - We may take some risks to save saveable property - We will not take any risks to save lives or property that are already lost |
Explain the safe person concept. |
Thinking about and acting safely to get the job done in a safe manner. |
What is dynamic risk assessment? |
- Identify hazards - Assess risk presented by the hazards - Identify options to reduce the risk - Decide if the risk is acceptable or not |
What are the three levels of safety you are responsible for? |
- Team (ensuring the safety of those you work with) - Task (doing the job safely) - Individual (personal safety) |
Referring to the safe person concept, describe the Task level of responsibility. |
- What does it take to do this job safely? - Have I been trained to do it? - What equipment do I need? - Do I need help? |
Referring to the safe person concept, describe the individual level of responsibility. |
- Being aware of hazards - Assessing risks - Being an effective team member - Working with equipment safely |
How is risk calculated? |
Risk=likelihood x consequence |
Thinking about risk, describe the five levels of consequence. |
- Catastrophic. Fatalities to staff, catastrophic loss of equipment (3 or more appliances. - Major. Major injuries (permanent disability), major loss of equipment (1 appliance) - Moderate. Serious injury (hospital), moderate loss of equipment (e.g. broken ladder) - Minor. Minor injuries, minor loss or damage to equipment (eg. standpipe knocked out from ground) - Insignificant. Insignificant injury or loss damage (e.g. burst hose). |
Thinking about risk, what are the levels of likelihood and describe them. |
- Almost certain. Is expected to happen, >90% chance. - Likely. Will probably happen. Between 70-90% chance. - Possible. Might happen. 30-70% chance. - Unlikely. Could happen. 10-30% chance. - Rare. Happens in exceptional circumstances. 0-10% chance. |
Referring to the risk calculation, describe the four levels of risk. |
- Very high. Do not proceed, - High. Close supervision required. - Medium. Normal procedures should suffice. - Low. Insignificant, monitor for escalation. |
