Supine with knee flex
>5 mm movement (low leg—on “pulling”/anterior) = injury (typically ant cruciate lig)
Excessive movement (low leg—on “pushing”/posterior) = injury (typically post cruciate lig)

Pt prone, knee flex, compress—internally/externally rotate
Pain on either side indicates meniscus injury
Apley’s compression test
Prone, knee flex 90, thigh stabilized, pull on ankle with internal/external rotation
Pain indicates nonspecific lig injury or instability (collateral ligs)
Apley’s distraction test
(for patellar dislocation)
Supine, legs extended, displace patella laterally
Apprehension on patient or contraction of quad mm indicates tendency to patella dislocation
Apprehension test
Supine with knee flexed—pt relaxes/drops knee
Inability to fully extend or “rubbery” feel on full extension = + sign
Bounce home test
(major knee joint effusion)
Press down above patella (+ = patella with elevate), then with other hand push patella “down” to femur (palpable tap of patella against femur = + sign)
Ballotable patella test
(minor knee joint effusion)
Bulge test
Supine with knee flex to ~30 degrees
Thigh stable “pull” ant on low leg
Excess movement = typically ant cruciate lig tear
More reliable to assess cruciate than drawer sign (<meniscal impinge and less hamstring tension)
Lachman’s test
(prone, knee flex 30, low leg moved post)
Posterior cruciate tear if excess movement
Reverse Lachman’s
Supine with knee flexed, internal/external rotation with extension
Palpable/audible click indicates meniscus injury
McMurray’s test
Supine, legs extended, move patella lat/med with downward pressure
Pain under patella indicative of chondromalacia patellae
Pain over patella indicative of prepatellar bursitis
Patella femoral grinding test
Collateral ligs
With some knee flexion also involves cruciate ligs
Valgus (abduction) / Varus (adduction) stress tests
Same as McMurray's:
