Contraindications To Live Vaccines

Pregnancy. HIV. Immunosuppressant therapy. Luekemia, lyphoma. Cellular immunodeficiency. SOlid organ transplant. Hematopoietic stem cell transplant.

Infulenza Vaccine

Inactivated quadravalent for everyone. Trivalent above 18. LAIV should not be given to those with contraindications to live viruses. Recombinant 18 years or older. Everyone should get flu vaccine once a year.

Egg Allergy and Influenza Virus

Can person eat eggs, then proceed with vaccine. If only hives than RIV3 if older than 18, if younder than 18 then IIV and observe for 30 minutes. If systemic reaction to eggs, then RIV3, if no RIV3 then IIV administer with supervision of physician.

Tdap

Should have 5 doses when child last at 4 to 6. If patient failed to complete as child, less than 3 doses, then 3 doses total of Td, one of which being Tdap. All people above 11 should receive one booster shot of Tdap, and all women 27-36weeks pregnant should get booster shot. Td should be given once every 10 years.

Varicella Vaccine

Before 1980 is documentation enough of chicken pox vaccine, unless pregnant, immunocompromised, or health care worker. After 1980 written proof of vaccine or infection. If no documentation then VZV vaccine 2 doses 4 to 8 weeks apart.

Zoster Vaccine

All patients above the age of 60, non immunocompromised.

PPV23 Recommendations

Adults >65, Adults 19-65 who have chronic hear disease, chronic lung disease, DM, ALcholism, chronic liver disease, smoking.

PPV13

Immunocompromised. Sicle cell, congenital asplenia, HIV, CKD, nephrotic syndrome, leukemia, lymphoma, hodgkin, generalized malignancy, Iaratogenic immunosuppression, solid organ transplant, multiple myeloma.

PPV23 and PPV13 Schedule at 65 Immunocompetent

If patient has not received PPV23 before, then give PPV13 then administer PPV23 1 year later. IF they have received PPV23 before 65 then administer PCV13 1 year after prior PPSV23, then administer PPSV23 1 year after PCV13. If received PPSV23 at or after age of 65 then administer PCV13 at least 1 year after.

Immunocompromised and Pneumococcal

Give Prevnar then 4 to 8 weeks later PPSV23. If already given PPSV23 then give Prevnar 1 year after.

HPV Vaccine

All females 11-12yo, or age 12-26 for unvaccinated females. Males 11-12 or age 13-21 unvaccinated. If immunocompromised or male homosexuals up to age 26.

MMR Contraindications

Pregnant women and immunocompromised

Meningococcal Vaccine

Given 2 two doses to asplenia or complement deficiences. Given in one dose to college students, who did not receive after age of 16, travleers to endemic areas, military recruits.

Hep A Vaccine

Anyone requesting, male male sex, travelers to endemic regions, illicit drug use, health care workers, chronic liver disease.

Hep B Vaccine

All those seeking vaccine. high risk sexual exposures, blood exposure, ESRD, HIV, chronic liver disease, travelers to endemic arease, DM2
