When can the FHR be detected by doppler ultrasound?
10-12 weeks
When can the FHR be detected by fetoscope?
18-20 weeks
When is the AFP test done?
15-21 weeks
When is the level II ultrasound done?
20 weeks
When should the woman start counting fetal kick counts?
26-28 weeks
When is the diabetes test done for mom?
28 weeks, earlier if at risk
What is the diabetes test for mom called?
OGTT (Oral glucose tolerance test)
-Mom is given 50g oral glucose
-Levels should NOT be above 120-135
What is the triple screen test?
Tests for AFP, unconjugated estriol, and hCG.
What does AFP test for?
Neural tube defects. Protein secreted by the fetal liver and excreted in the mother's blood.
What are the VDRL and RPR tests?
Test for syphilis. Needs to be tx early in pregnancy before fetal damage occurs. *Must recv tx before 16 weeks gestation.
When is the GBS test done?
32-36 weeks
What are the parts of the Biophysical profile?
Amniotic fluid volume
Fetal body movements
Fetal breathing movements
Fetal heart rate reactivity
Fetal muscle tone
How is the BPP calculated?
Each variable is given a max of 2 pts.
- 8-10 indicates healthy fetus
- 6 is suspicious
- 4 is in jeopardy
What does the Nonstress testing do?
Evaluates fetal well being by measuring the fetal heart response to fetal movements.
What position is the mom placed in for the nonstress test?
Semi-fowler or lateral tilt, pillow under one hip.
Why shouldn't the pt be placed in the supine position for testing?
Pressure on the maternal great vessels from the gravid uterus may cause maternal hypotension and reduced uterine perfusion.
What does a reactive NST show?
2 FHR acceleartions that exceed baseline by at least 15 bpm, last longer than 15 sec, occur w/i 20 min period
What does the CST test for?
Evaluates respiratory fn of the placenta
Indicates whether the fetus will be able to withstand the stress of labor
How are the contractions created for the CST?
Oxytocin-induced or Spontaneous
What would we like to find on the CST?
3 contractions, each lasting 40 sec, in 10 minutes. If FHR stays constant, the test is normal.
Late Deceleration
FHR drops below baseline at the end of the contx or after the end of the contx.
