Causes?

Bacteraemia = Strep Viridans / Staph Aureus / Staph Epidermidis / Haemophilus



Fungi = Candida


Other = SLE/ Malignancy

Clinical Features

Septic Signs? = Fevers/ Rigors/ Night Sweats/ Weight Loss/ Clubbing / Splenomegaly



Cardiac signs? = ANY NEW MURMUR



Immune complex deposition = Microscopic haematuria / AKI/ ROTH spots/ Splinter haemorrhages



Embolic Phenomena = Osler's Nodes/ Janeway lesions

Diagnostic Criteria?

2 Major + 1 Minor OR 1 Major + 3 Minor or 5 Minor


Major = Positive blood culture / Involvement of Endocardium


Minor= Fever / Predisposition (IVDU/Prosthetic valve)


Investigations

Blood Culture


Echocardiogram


ECG = > PR interval

Management

Blind + Normal Valve = Amoxicilin + Gentamicin


Blind + Prosthetic Valve = Rifampicin + Gentamicin + Vancyomycin



Staph + Native = Flucloxacillin


Staph + Prosthetic = Rifampicin + Gentamicin + Flucloxacillin



Strep = Benzylpenicllin + Gentamicin


Enterococci = Amoxicillin + Gentamicin

When to consider surgery?

Heart Failure


Valvcular obstruction


Repeated emboli


Fungal endocarditis


Persistent bacteraemia
