Infective Endocarditis |

Gentamicin |
Gentamicin is added to primary antimicrobial therapy for synergy when feasible for more difficult infections to eradicate such as prosthetic valve infections Duration of gentamicin varies based on the presence of prosthetic valve (2 weeks) or pathogen (4-6 weeks) When gentamicin is used for synergy, target peak levels of 3-4 mcg/mL and trough levels<1mcg/mL. Extended interval dosing is not used for AMG when treating endocarditis Rifampin - used in cases of staph prosthetic valve endocarditis due to its ability to treat organisms in a biofilm |
