Take a history of a headache

1. Onset, duration


2. Region/location: focal (retro orbital, frontal, occipital), unilateral, generalized


3. Temporal aspects: >50% of days; attacks of hours/days; attacks of up to 1 hour; many attacks, lasting minutes; attacks of seconds; worse on waking; new, acute/subacute onset; explosive onset, severe


4. Character & severity: tight band around the head, bland features, not very severe; throbbing, moderately severe; extremely severe, constant; severe, stabbing, lancinating


5. Provoking/relieving factors: provoked by alcohol; occur at night, start in sleep; relieved by sleep; triggered by touching or moving the face; caused by cough; exertion; orgasm


6. Associated symptoms: nausea, vomiting, photophobia, phonophobia; migraine aura (visual, sensory); neck stiffness, photophobia, vomiting, symptoms of fever; tear production Ipsilateral to a unilateral headache; Ipsilateral conjuctival injection; visual obscurations; persistent focal Neurological symptoms


7. General health: indications of systemic Neoplasia; polymyalgia & weight loss
