Nucleoside Reverse Transcriptase Inhibitors (NRTI)

*become part of HIV's DNA and causes damage
-Zidovudine (AZT, Retrovir)

s/e= anermia, bone marrow suppression, peripheral neuropathy, GI, pancreatitis
n/i= check labs, call doc if WBC dec- do not give drug : poses a risk to T Lymphocytes

*AZT can be given to pregnant woman 14-34 weeks
Non-nucleoside Reverse Transcriptase Inhibitors (NNRTI)

*affects enzyme necessary for conversion of HIV RNA into HIV DNA
-Nevirapine (NVP)

s/e= rashes, elevated transaminases, GI, CNS
n/i= liver: check ast, alt, if increasing liver damage is occurring

*safety is a big concern
Protease Inhibitor (PI)

*affect enzyme required for splitting of virus into particles that infect new CD4-T4 cells
-Nelfinavir (Viracept)

s/e= GI, elevated transaminases, hyperglycemia, fal maldistribution, lipid abnormalities
n/i= check ast/alt, need to monitor blood sugar at home- can cause insulin resistance (if bs is increasing and not a diabetic)
Entry Inhibitors

*acts during the attatchment stage of HIV cycle
Enfuvirtide (T20) - given SQ

s/e= local injection site reactions
n/i= given to people not GI tolerating with other meds, rotate sites!!
Integrase Inhibitors

* inhibits insertion of HIV into human DNA
Raltegravir

s/e= GI
CCR5 Antagonist

*prevents HIV entry into CD4+ cell
Maraviroc

s/e= GI, fever, diziness, elevated liver/ cardiac enzymes
n/i= check ast/alt and cardiac enzymes because this drug can cause damage to the liver and to the heart
PCP

* also given prophylactic T Cell Count < 200
TMP-SMZ (Bactrim/Septra)

s/e= rashes
MAC

*T cell count < 50 prophylactic
Biaxin, Zithromax
*biaxin can be given IVPB
TB
Isoniazid, Rifampin
Oral Candidiasis
Nystantin
Meningitis ( cryptococcus neoformans)
Amphotericin B (antifungal)

s/e, very severe= high fever, hypotension, tachypnea, generalized weakness
Cytomegalovirus Retinitis (CMV)

* prophylaxis TX T cell count < 50
Ganciclovir - severe neutropenia (must not be taking AZT)

Foscarnet- Renal failure, life threatening electrolyte imbalances
* once have this, need to stay on these drugs forever!!
Herpes Simplex, Herpes Zoster
Acyclovir
shortens disease length, but is not a cure!
