TPAL/FPAL |
Term births, preterm births, abortions and living children |
Gravida |
Heavy- pregnant woman Any pregnancy, # of pregnancies |
Primigravida primipara |
First pregnancy/One Given birth first child |
Nulligravida nullipara |
Never been pregnant Not given birth |
Multigravida multipara |
Been pregnant before Given birth to two or more children |
Diabetes Mellitus |
Pancreas doesn’t produce enough insulin to metabolize glucose normally. Type 1 insulin dependent. Type 2 non insulin dependent (diet, oral meds) |
Gestational diabetes |
Doesn’t produce enough insulin while pregnant. Increased glucose needs, cause hypoglycemia. R/f are age, existing DM, family hx, nonwhite race, obesity and HTN |
Gestation DM complications mom |
Infections, difficulty labor, vascular complications, c-section, keto acidosis, azotemia, hypertensive preeclampsia, |
Gestation DM fetal complications |
Stillbirth, spontaneous abortion, hydramnios(excessive amniotic fluid), large placenta, respiratory distress, congenital deformities, hyperbilirubinema, and hyperglycemia |
Sodium (pregnancy) |
Essential to maintain normal for plasma, bone, brain and muscle . SHOULD NOT be restricted during pregnancy but avoid high sodium foods(increase edema & HTN). Needed due to body water expanding. |
Wt gain/ sodium |
Major mineral controls extracellular fluid. Vital body water is increased during pregnancy 1.5-2.3g daily. Expanded blood volume and needs of fetus breastfeeding |
Breastfeeding |
Bonding, maintain infant temp, sucking stimulates oxytocin release. Mothers from different cultures use galactogogues ( beer, yeast, garlic, rice, gruel, fenugreek tea, and sesame tea). Every 2-3 hours normal schedule. |

Eastern Orthodox |
Baptized w/in 40 days. No birth control or abortions. Fasting meat & dairy wed & friday. Death no autopsy, no cremation no organ donation and euthanasia is not allowed. Christmas Jan 7 & new years Jan 14. |
Roman catholic |
Sick- forehead or hand. Birth unbaptized dont go to heaven so its mandatory. Contraception is prohibited & sterilization. No meat ash wed & fridays until lent is over. Rosary prayers pin to pillow in hospital. |

Pain mechanics affected by analgesic group |
Nonopoids-asa, Tylenol, and nsaids(tramdol, ibuprofen, naproxen, toradol, celebrex, Produced by inhibiting prostaglandins- primarily in CNS (pain/fever) but block pain impulses peripherally. Opioids-action in CNS bind to opoid receptors in brain & spinal cord. Narcan-opoid antagonists, reverse or blocks. Adjuvant analgesics-not understood. Ex-lyrica anticonvulsants used for pain, gabapentin anticonvulsant used for pain, antidepressants used for pain. |

Pneumonia |
S/s high fever, cough, pain, and headache. Other s/s lethargy, disorientation, anorexia, and low mild fever. Interventions: focus on liquidfying secretions by intake of fluids, meds, coughing, improved airway clearance and turing & deep breathing. |

Respiratory disorders considerations |
Encourage fluids, exercise, monitor smoking & second smoke, pursed lips breathing to improve oxygen, monitor o2, small meals reduce breathlessness, carbon dioxide retention-confusion, occipital headaches, and forgetfulness. Imbalanced nutrition is common (fatigue, dyspnea, and anorexia) |
Reducing falls |
Interventions-improving balance. Taught how to fall-rotate forward/backward to avoid falling sideways (less likely to obtain hip fx). Low vit D & calcium greater risk hip fx |

Risk for deficient fluid volume |
Fever, mouth breathing, fluid intake inadequate due to fatigue/dyspnea. Dehydration causes respiratory secretions to be thicker. S/s of DFV are decreased skin tugor, concentred urine, dry mucous membranes, elevated HGB & HCT. 3L fluid/daily |
Aspiration pneumonia |
Suction equipment, dysphasia-keep upright. External feedings-HOB upright, check tube placement, measure residual, right side if kept flat |

Pleurisy (pleuritis) |
Inflammation of pleura. Most common pneumonia, TB, injury to chest wall, pulmonary infarction, and tumors. Abrupt pain and occurs on one side of chest(pt points out) coughing & breathing aggravate it. Relief antitussives, antimicrobials, analgesics and local heat therapy. |
Acute hypoglycemia |
Too much insulin, not eating enough. Gastroparesis, renal insufficiency, meds (asa, beta blockers). S/s shaking, nervousness, irritability, tachycardia, anxiety, lightheadedness, hunger, tingling & numbness of lips/tongue, diaphoresis. Not caught 2nd set of s/s slurred speech, headaches, blurred vision, mood swings, disoriented, seizures, and unconsciousness. |
Treatments: OJ, apple juice, skim milk, sugar, honey, syrup, life savers 6-8, glucose gel |
Trigeminal Neuralgia 5th cranial nerve (tic doulourex) |
3 branches 1-controls sensation(eyes, upper eyelid, and forehead) 2-sensation to lower eyelid, cheek, nostril, upper lip & and upper gum 3-jaw, lower lip, lower gum, muscles used for chewing |
Older than 50-women more than me Facial pain suddenly intense spasms Triggered by dental work One sided- usually 2&3 branches |
Trigeminal Neuralgia-interventions |
Chronic pain syndrome-excruciating such a sharp shooting, piercing burning and jabbing. A typical is migraine like headache. Drug therapy carbamazepine, neurontin, lyrica and baclofen. Peripheral chemical nerve block percutaneous sterotactic rhizotomy -hollow needle through inside of pt cheek into trigminal nerves. Heating current destroys fibers or ballon microcompression. This is for long term pain relief. |

Terminal neuralgia-surgical management |
Microvascular decompression relocation of artery-may relieve TN pain. More invasive procedure requires craniotomy. Complications aseptic meningitis, cerebrospinal fluid leak, ataxia, ipsilateral (same side hearing loss) and facial nerve damage. Post craniotomy S/s of complications headache, cranial nerve dysfunction and bleeding. |

2.2lbs=1kg Pounds to kg divide by 2.2(smaller to bigger) 16oz=1lb Convert lbs to kg |
Child 65•2.2=29.54=29.5kg Adult 135•2.2=61.36=61.4kg |

Excessive CO2 in blood. Need to improve gas exchange |

Secretion from lungs-contains (mucous, cellular debris, microorganisms, blood or pus). Early am-best collection time(culture & sensitivity). |
