Plasmablastic lymphoma
Uncommon. Highest in HIV-infected individuals, m>f. Also other immunodeficiency states.
EBV-infected in majority.
Presents oral cavity>mucosal sites (orbit, sinonasal), skin, bone ,soft tissue, GI tract. Nodal involvement uncommon. Transformation of MM to PBL occurs (prior history of MM).
Looks immunoblastic (oral, head/neck) to plasmacytic (more in other sites, LNs)
CD138, 38+, CD45, 20, pax5-. CD79a+ in 50-85%. If CD56+ think underlying plasma cell myeloma. Ki67 usually >95%. EBER+ in 60-75%. HHV8 negative.
Prognosis bad, most die <1yr. HIV management?
