Who it is, what the injury/medical emergency is, who is making the phone call, what medical facility they are going to, phone number, and what they were doing at the time of injury.
What do you log if the boat is taking on fuel or ammunition?
Drafts before and after transfer, times of transfer, who is taking part, who is deliviring, type of fuel or ammo. You should also hoist the bravo flag and extinguish the smoking lamp.
What do you log for a fuel spill?
Location, type of fuel, how much is spilling, who is responding.
What do you log if the CO is going/coming off of leave?
Who is acting CO, days and times of leave, contact information.
What do you log for the changing of OOD's?
Time of change, who is relieving, and who is breaking in.
What do you log for absentees?
Who is absent, how long they have been absent, who contacted them, and what time they were contacted.
What do you log for restricted personnel?
How long for restriction, who is restricted, and for what reason.
What do you log for a captain's mast?
Date and time, who is involved, who is being masted, and where it is being held.
What do you log for a person reporting or departing?
Name and rank, reporting from, and EMPLID
What to do in an emergency?
Make proper emergency pipe and proper alarm. Give the location of the emergency and what kind of emergency it is. Tell what lockers to provide from. SOPA makes official call of what actions will be taken before and after the emergency.
