2. Knock, set up. Privacy curtain. Wash hands. Inside supplies: Soap, lotion, gloves. Get basin of water, wash hands, check temp. Gloves.


3. Change bed pad. Smooth out.


4. Dip washcloth, add soap to washcloth. Middle-C-C. Turn. Give time to rest. Wash back.


5. HOB up, lower bed, new water. Remove gloves, wash hands, check temp.


6. Rinse, pat dry.


7. Change pad. Lower bed. Clean up. Call light. Open curtain. Wash hands.

Bed pan

1. Knock, intro, wash hands. Supplies: 2 bed pads, bags, washcloths, barriers, TP.


2. Knock, set up. Privacy curtain. Wash hands. Bed up, HOB down.


3. Change bed pad.


4. Stick bed pan under, ask if comfortable. Put HOB up, close curtain, step away.


5. HOB down, bed up. Slide pan out. ask if he wants to wipe. Wipe bottom, throw in pan.


6. Change pad, wipe out wrinkles.


7. Ask if he wants to wash hands with moist washcloth.


8. Bring HOB up, bed down. Call light.


9. Dump pan into toilet. Clean up. Wash hands.

Hand and nail

1. Knock, intro, wash hands. Supplies: Clean/dirty bag, washcloth, barriers.


2. Knock, set up. In room: Orangewood stick, nail file, basin, nail clipper, nail brush, lotion. Curtain. Wash hands.


3. Get water in basin, wash hands, check temp.


4. Let him soak hands. Make hand mit. Pat dry.


5. Clean under nails with flat part of orangewood stick.


6. ASK if diabetic. Clip nails, file, brush off. Ask if comfortable


7. Offer (warmed) lotion on hands.


8. Dump basin, rinse, dry.


9. Clean up, call light, open curtain, wash hands.

Measuring contents of urinary drainage bag

1. Knock, intro, wash. Supplies: bags, barriers.


2. Knock, set up. In room: Grad. container, wash basin, gloves, pen, scratch paper, alcohol preps (2)


3. Put bed up


4. Place barrier, basin, grad container on floor


5. Swipe with alc. prep. Drain, close. Swipe again. Read at eye level.


6. Dump in toilet (announce), rinse, dump, rinse basin.


7. Clean up, remove gloves, open curtain, wash hands.


8. Document.


9. Bed in lowest position, HOB up.

Folley catheter

1. Knock, intro, wash. Supplies: bags, washcloths, bed pads, 2 towels.


2. Knock, set up. Get gloves, lotion, soap, basin. Curtain.


3. Wash hands, fill basin, check temp, gloves.


4. Bed up, HOB down.


5. Change bed pad.


6. Make hand mitt, check temp, soap to washcloth. Do wash strokes - around insertion, anchor + 3/4 inches down tube, c, c.


7. Turn over. Wash back.


8. Lower bed, raise HOB


9. Get new water. wash hands, check temperature.


10. Put bed back up, rinse, dry pat back.


11. Replace bed pad, smooth out wrinkles.


12. Dry pat front.


13. HOB up, bed down, call light, back rub, curtain, wash hands.

Denture care skill

Supplies: bag, barriers, clothing protector, dentures, denture cup with lid, adhesive, tab, denture brush, oral swab, moutwash, emesis basin, chapstick, little + big cup, gloves, lotion.



Keys: must offer clothing protector, ask about pain discomfort, make sure water is cool/tepid, offer pt to rinse and spit, 50/50 water/mouthwash, offer chapstick, carry dentures in cup with lid, offer backrub, pad sink.

Dressing

Knock, intro self, pull curtain, wash hands.



Dress weak side first. Ask and prepare what he wants to wear. Grippy socks.


Pants: gather, slowly lower legs. Stop, turn. Pull up rest of way. TUCK IN POCKETS. Repeat


Shirt: Bad side first. Arm, head, straighten back.



Call light, curtain, gloves, wash hands.

Pulse

RADIAL pulse with middle+index finger near thumb side. SUPPORT hand. Count for one minute.
