Describe the continuim of healthcare from acute care to independent living.
ICU is for medical emergency. Once stablized the pt is sent to the floor of the hospital to reach or continue homeostasis. Subacute or Transitional Care is like a rehab. Under Medicare client may stay for up to 20 days; usually the client stays for one to four weeks.

SNF, skilled nursing facility, provides 24 hour care and must always have a licensed nurse on duty. This is for a pt that needs skilled specialized wound care or two more weeks of antibiotics.

Assisted Living for people who have Alzeimers /forgetful or need assistance with ADL's.

Long-term care or congregate housing are nursing homes.

Independed Living is at home but has assistance of home health nurse
Differentiate among the major types of long-term facilities
Long-term Care Facilities are called nursing homes and are expected to provide clean, safe and home-like environments. They provide room, board and a range of nursing care. Emergency medical care is also available. Two Types:

SNF, skilled nursing facility, provides 24 hour care and must always have a LPN or RN on duty. SNF provides rehabilitation services, special diets, and access to pharmacy and laboratory services. Occupational, physical and speech therapists are usually on staff or serve as consultants. Podiatary and dental services, social services are often provided.

Intermediate Care Facility provides fewer services and less extensive care than SNF: room and board, and some nursing care. Nurse not on duty but on call.
