Steps to CM |
1. Defining the problem(s) -start by gathering information: -screen -intake: paperwork(TCF, ROI, Insurance, Confidentiality) -assesment(core function): information about the client -micro -mezzo -macro 2. Determining severity of the problem: - developing hypothesis 3. Figuring out why problems are happening 4. Establishing Goals (S.M.A.R.T) Goals should be: -behavioral - community services - support systems - problem-solving skills clients should acquire 5. TP- the process where the counselor identifies problems, establishes goals, and decides on the treatment method and resources 6. Evaluation - determining the degree of goal achievement How much does your client's success depend on you- it depends 7. Termination: a process not an appointment -client has achieved goals -proven they can self manage - successfully working w/ liaisons *In this stage: -take a stroll down memory lane -talk about case manager- client relationship 8. Follow-up -time and money are often a hindrance |
