levodopa-carbidopa (Sinemet)

Dopamine replacement



Indications: control symptoms of Parkinson’s. Carbidopa isn’t used to treat symptoms, but the nausea associated with Sinemet



Usual dose: anywhere from 3-8 times a day, others at 4-8 hour intervals



Adverse effects: GI effects, nausea



Contraindications: angle closure glaucoma, undiagnosed skin condition because can cause malignant melanoma



Interactions: tricyclic antidepressants, antipsychotics, nonselective MAOIs, benzodiazepines (antipsychotics)



Administration: best taken on an empty stomach but can be taken with food to prevent GI upset. Sinemet CR cannot be crushed but can be split one time

selegiline (Eldepryl)

Indications: Parkinson’s, adjunct for levodopa



Usual dose: 5 mg bid w/ breakfast and lunch



Adverse effects: weight loss, GI upset, back pain, impulse control disorders, headache, confusion



Contraindications: meperidine, allergy



Interactions: tyramine containing foods, meperidine



Administration: can be used with levodopa or monotherapy

ropinirole (Requip)

Nonergot NDDRA



Indications: early and late stage Parkinson’s, restless leg



Usual dose: 0.25 mg tid, slowly increasing to max dose of 24 mg/day



Adverse effects: dizziness, GI upset, somnolence



Contraindications: allergy



Interactions: any drug metabolized by cytochrome P-450 enzyme 1A2



Administration: oral use only, delay the need for levodopa therapy

amantadine (Symmetrel)

Dopamine modulator



Indications: influenza virus infections, mild to moderate Parkinson’s



Usual dose: 100-400 mg/day divided every 12 hrs



Adverse effects: dizziness, insomnia, nausea



Contraindications: allergy



Interactions: anticholinergics



Administration: oral use only

benztropine (Cogentin)

Anticholinergic



Indications: Parkinson’s, extrapyramidal symptoms from antipsychotics



Usual dose: 0.5-6 mg/day



Adverse effects: hyperthermia, tachycardia, confusion, disorientation, toxic psychosis, urinary retention, dry mouth, nausea



Contraindication: use with caution in hot weather and exercise



Interactions: alcohol, other drugs with anticholinergic effects



Administration: tablets and injectable

tolcapone (Tasmar)

COMT inhibitor



Indications: Parkinson’s



Adverse effects: sever liver failure, GI upset, urine discoloration, worsened dyskinesia



Contraindications: allergy, liver failure



Interactions: no selective MAOIs



Administration: rarely used

entacapone (Comtan)

COMT inhibitor



Indications: Parkinson’s



Usual dose: 200 mg with each levodopa dose, max of 1600mg/day



Adverse effects: GI upset, dyskinesia, urine discoloration



Contraindications: allergy, liver disease



Interactions: nonselective MAOIS



Administration: oral use, available in combination tablets
