For injury reporting the employee may file a pre-selection of personal physician form

This form is called #FD-080

For off the job injuries medical details do not need to be shared.

However the employee must report it to his or her supervisor. And must enter the absence in telestaff

Within 24 hours of knowledge of the incident, the supervisor is required to provide the battalion chief with all appropriate documentation.

This will include a DWC form 1 and the employees claim for Worker’s Compensation benefits.

Supervisor responsibilities

The division chief of administration should be contacted within 24 hours of an injury.

They need to ensure that all appropriate forms including form 5020 from the workman’s compensation packet, are completed and submitted within 72 hours of the original incident.

Battalion chief responsibilities

The division chief of admin will bring the Worker’s Compensation claim to risk management if appropriate and inform the other chiefs of the injuries

For on the job injuries if no medical attention is required the employee and their supervisor shall complete the report of injury or exposure form to the chief of admin

This should be done within five days of the date of injury

If an on the job injury does occur and needs medical attention the employee will be sent to the most appropriate facility.

When leaving the doctor the employee should have the following in hand

Prescriptions if needed, a work status note from the doctor, form RM1010 “medical referral form“. All notes should include the following info: work status, work limitations, disposition, physician signature and date.
