S/s: localized tissue necrosis


Small bleb (blister) surrounded by white ring


Most pts are unaware that they got bit until area becomes ischemic


Tx: no specific prehospital Tx

Black widow s/s and Tx:

Pain


Progressive muscle spasms


Rigid abdomen


Seizures/ paralysis


Tx: to reduce muscle spasms give versed 5mg slow IV or 10mg IM/ IntraNasal


If pt becomes agitated give morphine 5mg iv/im Max 20mg

Scorpion stings s/s and Tx:

Sharp pain progressing to numbness


Salivation


Slurred speech


Muscle twitching


Tx: pain management and transport

Marine animal envenomations

Immerse puncture in non-scalding hot water (110-113F) to achieve pain relief, continue immersing urging transport


Don’t remove spine in field


Pain management


May take 30-90 minutes for relief with water

Sting rays, scorpion fish, catfish, lion fish etc.

Marine animal stings s/s and Tx:

Evaluate ABCs


Attempt to remove visible tentacles with 4x4 with a double gloved hand


If available apply a vinegar soaked gauze to area for 30 minutes


Pain management


After pain relief attempt to remove remaining tentacles with shaving cream and gentle scraping with credit card

Jellyfish, man-o-war, sea lice , hydroids or fire coral

Bees/wasps/hornets/yellow jackets/ ant stings s/s and Tx:

Pt will have pain or allergic reactions


Tx: apply local ice packs
