Coordination of care::: is when the nurse coordinates the care delivery.



Health teaching and health promotion:::: is when the nurse use strategies to promote health and safe environment.



Consultation :::: is when the nurse provides consultation to influence the identified plan.



Prescriptive authority and treatment:::: is the use of prescriptive authority and therapies in accordance with the state and federal laws and regulation.

What is context of care ?

Context of care is a situation associated with an event....... All the types of assessment are in our context of care.

Explain the ff?


Comprehensive assessment, problem based/focused assessment , episodic/follow up, shift, screening.

Comprehensive assessment is when the nurse do a detailed physical examination on a pt at the onset of care in a hospital, long term facility.



Problem based/ focused assessment:::: this refers to history and examination that are limited to a specific problem.



Episodic/follow up:::: is usually the pt following up with a health care provider.



Shift::: is when nurses conducts assessment on each of their various shift.



Screening ::: is a short assessment that is focused on disease detection.

What is data organization ?

Data organization is done after collecting pt data by the nurse so that pt problem will appear more clear.

What is data analysis, interpretation and clinical judgement ?

Is when data is organized to help find the problems and initiate an appropriate plan of care.

What is Clincal manifestation ?

Is used to describe presenting signs and symptoms experienced by the pt.

What are the techniques for physical assessment ?

IPPA::::


Inspection ::: involves visual examination of the body including body posture and movement.



Palpation:::::using your hands to feel for texture and shape and also where pt report of experiencing pain there.



Percussion::: is done to evaluate the size borders and consistency of internal organs.___ direct percussion= is striking your fingers direct on the pts body.____indirect percussion= is when u use both hands and is done by methods depending on the specific part of the body that is being assessed.



Auscultation is listening to the sounds within the body.

What part of the hands are used to palate ?

Finger pads, palmar surface and ulnar surface.

Deep and light palpation

Light ==1cm and used for skin assessment, pulsation and tenderness



Deep==4cm and used for organ size and contour .

What are the functions of diaphragm and bell of the stereoscope ?

Diaphragm ==high pitch sound like heart beat, bowel, and breathe.



Bell==is used for low pitch sound like extra heart sound and vascular sound.

Where are these sounds heard from ? Resonant, flat, dull, tympanic, hyperesonate

Resonant: heard over healthy tissues



Flat==bone and muscle



Dull==liver



Tympanic== abdomen/stomach



Hyperesonate ==inflated lungs.

What are the techniques that enhances data collection ?

Clarification(obtaining more information abt conflicting or ambiguous stuffs)




Active listening



Restatement(repeating what the pt said to confirm interpretation of what was said)



Facilitation.(using phrases like go on, then .... To encourage the pt to tlk)
