217-01 “vehicle collision-general procedure”


When do you give preliminary breath test (PBT) to operator ?

UMOS will give PBT If you have SPI (serious physical injury) and STV (serious traffic violations)


UMOS DUTIES:


**if property damage only:


-umos will provide collision info exchange form/what you should know about veh collision card and mv104 to parties involved


-inform motorist they must prepare the mv104 within 10 days of veh collision


-radio code 10-99T3 (refer to DMV)


-place code at least 200ft from collision


-no ins required for BIG FART :


B- buses


I- ICC


G- govt owned


F- farm veh (certain veh)


A- auth of public service commissions


R- rented veh


T- taxis


What is a qualifying vehicle ?


-comm veh weight over 10,000


-veh with hazmat placard


- buses with 9 or more passengers (including driver)


*prepare mv104S if


- at least 1 qualify vehicle & at least 1 condition of severity is present (person is fatal/transported to hosp/veh tow)


-each qualified veh gets their own mv104S


-fax copy of PAR involving crossover to corporation counsel and the Highway district


-send copies of PAR marked “duplicate copies for” to appropriate agency in an white envelope and forward via mail and distribution unit


-give PAR to D/O of pct of occ to sign PAR



Desk officer duties :


-sign all PAR in forms and handwritten PAR as well

217-02 “veh collision which result in death, serious injury and likely to die or critical injury “

-pct det


-Highway Dist collision technician group


-Highway Dist collision investigation sqd


UMOS DUTIES :


-detain all involve parties


-if motorist is dwi, Highway Dist personnel will do PBT


-notify P/S to respond


Patrol Supv duties:


-notify desk officer and CO /DC


-notify operation unit if and extended closure of a roadway is required or if perishable merchandise or current newspaper are involved


-if mos is involved and suspected of dwi immediately notify CO/DC

217-04 “accident and collision- city involved”


Claims against the city must be filled with the city within ?

Within 90 days


UMOS duties:


-either prepare PAR or AIDED or City involve report (only 1 report out of 3 will be prepared)


-submit the report to D/O


Desk duties :


-review and sign report, as appropriate


-forward PAR in normal manner

****217- 06 “department vehicle collision “


Who will be investigating Supv if DOA or serious injury to anyone?


Cop- does mv104


P/S - does mov104AN and mv104L and possibly CRPDV(if he is the investigating Supv)


Investigating Supv - does collision report- police dept veh (CRPDV) part A

Pct CO/DC (will complete part A)


UMOS DUTIES:


*if umos is not assigned to pct of occ, than his/her Supv will perform the duties of p/S, if available


-prepare mv104


-P/S will determine if a summons be issued to a pvt veh but NEVER TO DEPT VEH


PATROL SUPV DUTIES:


-respond and assume role as investigating Supv if only property damage or non-serious injury cases


-notify D/O of details


-notify operations unit promptly if mos is injured and require hospital treatment (additional notification required as well to update the status of mos in hospital)


-prepare mv104AN (always will be prepared) via forms in quadruplicate


-prepare and sign mv104L via form


-enter member command and address of that command under caption operator address


-print in large “police involved” at top of the form under caption “police accident report”


-have 2 sets of photo taken of dept veh showing damage and be secured In jewelry security envelope (D/O will forward 1 sets to pct XO with PAR and collision report. And 1 sets to boro service station with PAR if collision involved death or injury)


-if umos was operating the veh during an “emergency operation”, enter letter P in a circle nxt to the operator name


-deliver 1 completed copy of PAR to operator of dept veh involved in collision


Investigating Supv duties:


-respond and conduct canvass for witness, videos


-interview all persons involved


-if video found, voucher as “investigatory evidence”


-notify pct det for assistance, if video is available for access. If pct det can’t retrieve the video than notify TARU


-prepare part A of collision report


-if mos at fault, include in collision report- dept veh


-ensure the cause of the collision is recorded on PAR and collision report


-sign and deliver the complete report to D/O.


-send a copy of CRPDV to CO, risk assessment unit, if any video has been invoice


Desk officer duties:


-notify CO and patrol boro


-request DC, when applicable


-enter the boro collision # into Tele record


-notify fleet service Div and obtain collision serial # (enter the # in 2 places. at top of the PAR and in designated box on “collision report-police dept veh”)


-fax mv104 and mv104L and collision report to the personnel safety desk on the tour that the collision occurred, if possible


-send the veh to the shop with photos


-forward original PAR TO TRAFFIC SAFETY OFFICER for review and processing


-XO endorses all reports
