Ventricular tachycardia: symptoms

-may be asymptomatic


-lightheaded or syncope


-occasional anginal pain, dyspnea, or CHF



*if untreated can heart will fail*

nonsustained:


-not tx unless symptoms are significant



sustained (>30 beats):


-DC cardioversion (Shock)


-antiarrhythmic (use cautiously if LV dysfxn or weakened heart)


-implantable defibrillator


-ablation


-if ischemic disease- use beta blockers



(avoid stimulants & remove any offending agents)

What pathology?


-grossly irregular waveforms w/ varying morphology


-rate rapid & variable (irreg)


-indistinguishable QRS complex

Ventricular Fibrillation: symptoms

-sudden syncope & pulselessness


-will rapidly lead to death if not converted


(need shock, doesn't convert sponataneously)

-ACLS protocol--> DC cardiversion (shock), CPR, antiarrhythmics


-implanted defibrillator (shock)
