Causes of Ventricular Arrhythmias |
ischemia cardiomyopathy ventricular scarring/aneurysm** toxins/drugs (meth or cocaine) pre-excitation dysplasia |
|
T/f Ventricular arrhythmias can be lethal |
TRUE
(esp if drug uses, meth, coke, etc)
|
|
Ventricular arrhythmias: symptoms |
VERY FEW (usually none) -palpitations -syncope (^ if a lot may cause to pass out) |
|
Ventricular arrhythmia: tx options |
- avoid stimulants - beta blockers help w symptoms - usually no intervention needed |
|
HR 120-180 wide complex QRS no P wave
what pathology? |
Ventricular tachycardia
(if lasts for > 30 beats = sustained= high risk of death, less = unsustained) |
|
Ventricular tachycardia: symptoms |
-may be asymptomatic -lightheaded or syncope -occasional anginal pain, dyspnea, or CHF
*if untreated can heart will fail* |
|
Ventricular tachycardia: tx |
nonsustained: -not tx unless symptoms are significant
sustained (>30 beats): -DC cardioversion (Shock) -antiarrhythmic (use cautiously if LV dysfxn or weakened heart) -implantable defibrillator -ablation -if ischemic disease- use beta blockers
(avoid stimulants & remove any offending agents) |
|
What pathology? -grossly irregular waveforms w/ varying morphology -rate rapid & variable (irreg) -indistinguishable QRS complex *VERY weird EKG! |
Ventricular Fibrillation |
|
Ventricular Fibrillation: symptoms |
-sudden syncope & pulselessness -will rapidly lead to death if not converted (need shock, doesn't convert sponataneously) |
|
Ventricular Fibrillation: tx options |
-ACLS protocol--> DC cardiversion (shock), CPR, antiarrhythmics -implanted defibrillator (shock) |
