how do you name the cranial strain patterns?
position or orientation of the sphenobasilar syncondrosis
what are the four physioloic strain patterns?
flexion, extension, torsion, side bending rotation
what are the three non physioloic strain patterns?
lateral strain, vertical strain, sphenobasilar synchrondrosis compression
what are the etiologies associated w/ Flexion and extension strain patterns? theaxis? how do you diagnose them?
etiology: birth, genetic
axis: two transverse axis
diagnoses: named for the position of the SBS (flexed or extended)
what motion is present, but diminshed in a flexion strain pattern?
extension motion
what does an extension strain pattern cause?
depressed SBS
A-P diameter is increased
Transverse diameter decrease
motion preference is extension/ internal rotation
sacrum nutated
what motion, while present, is diminished in an extension strain pattern?
flexion
what would an observational diagnosis of a flexion strain pattern include?
face round, wide side to side, short from above down
orbits widened diagonal diameter, eyes protruded
ears protruding
palate wide and flat
what would an observational diagnosis of a extension strain pattern include?
face oval, narrow side to side, long from above down.
orbits narrow diagonal diameter, eyes retruded
ears close to head
palate narrow and high
what would you feel on the classic vault hold w/ a flexion strain pattern?
fingers on the greater wings of the sphenoid and lateral angles of the occiput are symmetrically widened and somewhat inferior.
what would you feel beneath your fingers in the classical vault hold during an extension strain pattern?
fingers on the greater wings of the sphenoid and lateral angles of the occiput are symmetrically narrowed and somewhat superior
what are the etiologies of tosion?
birth, trauma, typically at an angle to one quadrant of the head.
a blow in the anterior quadrant would affect what two bones?
frontal and zygoma
a blow in the posterior quadrant would affect what two bones?
parietal or occipital
what is the axis for torsion?
anterior-posterior axis from nasion to opisthion
what two bones at the SBS rotate in opposite directions?
sphenoid and occiput
how do you name the side of torsion?
named for the side of the superior (high) greater wing of the sphenoid bone
Using the vault hold, what would your left and right hand experience in a left torsion?
left hand: index finger is superior (rotates towards you); little finger is inferior (rotates away from you)
right hand: opposite of the left
Using the vault hold, what would your left and right hand experience in a right torsion?
Left hand: opposite of the right
right hand: index finger is superior (rotates towards you); little finger is inferior (rotates away from you)
what is the etiology of Side bending/ rotation?
trauma

blow to left side--> right side bending rotation strain
opposite for right
what are the axes of sidebending/ rotation? which way do the sphenoid and occiput rotate in each axis?
Vertical axes: 2 parallel
one through the center of the sphenoid
one through the foramen magnum
sphenoid and occiput rotate in opposite directions
one A-P axis:
opisthion (occipital bone at birth) to nasion (rotation)
sphenoid and occiput rotate in the same direction
how do you diagnose side bending/ rotation?
named for the side of the convexity of the SBS
what would you feel in the vault hold for a left SBR?
left index and little fingers spread apart and move inferior (away from you)

right index and little fingers come together and move superior (toward you)
what would you feel in the vault hold for a right SBR?
left index and little fingers come together and move superior

right index and little fingers spread apart and move inferior
what is the etiology of lateral strain?
trauma to one side of the head anterior or posterior to the SBS, or birth or from compression in utero
what are the axis for lateral strain? what other strain pattern has 2 similar axes?
2 parallel vertical axes:
1) thru base of the sphenoid
2) one thru the foramen magnum

sidebending has those axis + additional A-P axis.
how do you diagnose a lateral strain?
direction of the base of the sphenoid
what are the palpatory findings of a left lateral strain in the vault hold?
index fingers shift laterally to the right and the little fingers shift to the left
what are the palpatory findings of a right lateral strain in the vault hold?
index fingers shift laterally to the left and the little fingers shift to the right
what is the lateral strain clinical correlation?
plagiocephaly--> parallelogram shaped head
what is a vertical strain usually secondary too?
trauma
what are the two axes of vertical strain?
two parallel transverse:
- one thru the sphenoid
- one thru the occiput
how do you name the diagnosis for vertical strain?
named for the direction the posterior surface of the body of the sphenoid has shifted (superior or inferior).
what direction do the sphenoid and occiput rotate in for vertical strain?
same direction--> one in flexion and the other in extension
what palpatory findings would you find on a vault hold of a superior vertical strain?
index fingers rotate inferior and little fingers rotate superior
what palpatory findings would you find in a vault hold of an inferior vertical strain?
index fingers rotate superior and little fingers rotate inferior
pt presents with a hx of concussion, presenting with headache, low energy and a short attention span.. what should be highest on your list for strain pattern? what else would add more evidence to this differential?
SBS compression

- hx of head trauma, difficult labor (FORCEPS), pt w/ difficulty concentrating and sluggish thinking.
what two instances should you suspect cranial strains?
trauma and difficulty during birth
