MECHANISMS: Hiatal Hernias
|
People after middle age
-> Weakening of diaphragm muscle + Widening of oesophageal hiatus -> Protrusion of top part of stomach through oeso hiatus into mediastinum |
|
Hiatal Hernias:
Most common in what population? |
People after middle age
|
|
Hiatal Hernias:
Presentation? |
Distressful and painful
|
|
Hiatal Hernias:
2 main types? |
Sliding (95% cases)
Rolling (paraoesophageal) (5% cases) |
|
Hiatal Hernias: Sliding
Describe generally. |
Abdominal part of oeso + whole top circumference of stomach (cardia + fundus) slide superiorly thru oeso hiatus, into thorax
Sliding occurs when lying down or bending over Regurg possible (diaphragm crus clamping action weak) |
|
Hiatal Hernias: Rolling
Describe generally. |
Cardia (lesser curvature) stays still
Pouch of peritoneum + part of fundus -> Extends thru oeso hiatus (anterior to oeso) No regurg (cardia remains in normal position) Less common, but higher chance of strangulation Less |
|
Congenital diaphragmatic hernias:
Complications? |
Can lead to pulmonary hypoplasia (herniated abdo contents in pleural space impedes lung development)
Mortality 76% |
