MECHANISMS: Hiatal Hernias
People after middle age
-> Weakening of diaphragm muscle
+ Widening of oesophageal hiatus
-> Protrusion of top part of stomach through oeso hiatus into mediastinum
Hiatal Hernias:

Most common in what population?
People after middle age
Hiatal Hernias:

Presentation?
Distressful and painful
Hiatal Hernias:

2 main types?
Sliding (95% cases)

Rolling (paraoesophageal) (5% cases)
Hiatal Hernias: Sliding

Describe generally.
Abdominal part of oeso + whole top circumference of stomach (cardia + fundus) slide superiorly thru oeso hiatus, into thorax

Sliding occurs when lying down or bending over

Regurg possible (diaphragm crus clamping action weak)
Hiatal Hernias: Rolling
Describe generally.
Cardia (lesser curvature) stays still
Pouch of peritoneum + part of fundus
-> Extends thru oeso hiatus (anterior to oeso)

No regurg (cardia remains in normal position)

Less common, but higher chance of strangulation

Less
Congenital diaphragmatic hernias:

Complications?
Can lead to pulmonary hypoplasia (herniated abdo contents in pleural space impedes lung development)

Mortality 76%
