Adenosine MOA

Antidysrhythmic used for the tx of paroxysmal SVT.



Is a nucleoside that depresses conduction through the AV node.


This interrupts re-entry circuits within the heart and may restore SR in pts with SVT.

Adenosine INDICATIONS

Patients 12 yrs and older with SVT and ventricular rate 150/min and over AND


- Causing moderate cardiovascular compromise, or


- Known to be responsive to Adenosine.

Adenosine CONTRAINDICATIONS

1. Known severe Ax


2. Known sick sinus syndrome, without internal pacemaker in place (may cause severe bradycardia).


3. Previous 2/3° HB without an internal pacemaker in place (may cause HB).


4. Previous heart transplant without an internal pacemaker (following heart transplant the heart is denervated and adenosine may cause bradycardia).

Adenosine CAUTIONS

1. Asthma. May cause bronchospasm and should be withheld if pt has had recurrent life-threatening attacks.


2. COPD. As above.


3. WPW syndrome if the rhythm is possibly fast AF. Could precipitate VF.

Adenosine IN PREGNANCY

Safety not demonstrated. Balance of risk is in favour of administration.

Adenosine DOSAGE

6mg.



Second dose of 12mg if rhythm fails to revert.

Adenosine ADMINISTRATION

Undiluted as rapid IV bolus, rapid flush of 20ml NaCl (pref antecubital fossa vein).

Adenosine ADVERSE EFFECTS

1. Bradycardia


2. SOB


3. Light-headed


4. Nausea and flushing


5. Chest pressure and/or severe apprehension

Adenosine PHARMACOKINETICS

Rapidly taken up and metabolised within seconds by the RBCs and vascular endothelial cells.

Adenosine PREPARATION

Ampoule 6mg in 2ml

Adenosine COMMON INTERACTIONS

Dipyridamole** inhibits the cellular uptake of adenosine may cause duration of effect to be prolonged.



** inhibits thrombus formation, rarely prescribed.

Adenosine ADDITIONAL INFO

Usually causes brief period of very low cardiac output, often causing severe apprehension/sense of impending doom. Reassure pt this will pass quickly.
