Methylprednisolone Sodium Succinate (Solu Medrol)

CLASS: Corticosteroid
ACTION: Reduces inflammation by multiple mechanisms INDICATIONS: Anaphylaxis, asthma, COPD. ADVERSE: Depression, euphoria, HA, restlessness, HTN, bradycardia, N/V, swelling, diarrhea, weakness, fluid retention, parsthesia CONTRA: Cushings syndrome, fungal infection, measles, varicella, known sinsitivity ( including sulfites). Caution in active infections, renal disease, penetrating spinal cord injury, HTN, seizures CHF. DOSE: ASTHMA & COPD: Adult: 40 - 80mg IV Ped: 1mg/kg (up to 60mg) IV, 10/day in two divided doses. ANAPHALYCTIC SHOCK: Adult: 1 - 2mg/kg dose, 0.5 - 1mg/kg q 6hr Ped: Same as adult dosing BLUNT SPINAL CORD INJURY: Adult / Ped: 30mg/kg IV, 10 over a period of 1 hour. As infusion to run for remaining 23 hours add a dose of 5.4mg/kg/hr |

Hydrocortisone Sodium Succinate (Cortef, Solu-Cortef)

CLASS: Corticosteroid
ACTION: Reduces inflammation by multiple mechanisms, as a steroid, it replaces that are lacking in adrenal insufficiency. INDICATIONS: Adrenal insufficiency, allergic reactions, anaphylaxis, asthma, COPD ADVERSE: Leukocytosis, hyperglycemia, > infection, < wound healing, > rate of death from sepsis, CONTRA: Cushings syndrome, Known sensitivity to benzyl alcohol. Caution in DM, HTN, CHF, Known systematic fungal infections, renal disease, Idiopathic thrombocytopenia, psychosis, seizure disorder, GI disease, Gaucoma, Known sensitivity. DOSE: ANAPHYLACTIC SHOCK: Adult: 100 - 500mg IV, IO, IM Ped: 2 - 4mg/kg/day IV, IO, IM (max 500mg) ADRENAL INSUFFICIENCY: Adult: 100 - 500mg IV, IO, IM Ped: 1-2mg/kg IV, IO, IM ASTHMA / COPD: Adult: 100 -500mg IV, IO, IM Ped: 1mg/kg IV, IO. May be reduced for infants and children, but is governed more by severity of condition & response of pt than by age or body wt. Dose should not be <25mg/day. |

ACTION: Binds / blocks H1 histamine receptors INDICATIONS: Anaphylactic reactions ADVERSE: Drowsiness, dizziness, HA, excitable state (children), wheezing, thickening of bronchial secretions, chest tightness, palpitations, hypotension, blurred vision, dry mouth, N/V, diarrhea. CONTRA: Acute asthma, which thickens secretions, pts with cardiac Hx, known sensitivity DOSE: Adult: 25 - 50mg IV, IO, IM Ped (2 - 12yr): 1 - 1.25mg/kg IV, IO, IM |
