Define an aneurysm |
Abnormal focal dilatation in a blood vessel to >50% of its original size |
At what size do you operate on an Infra-renal AAA? |
5.5cm |
What is a pseudoaneurysm (false aneurysm)? What causes them? |
An aneurysm which does not involve all 3 layers of the vessel wall, often only lined by surrounding connective tissue or adventitia. Often secondary to penetrating trauma, iatrogenic (surgery, femoral cannulation), or infection |
Aneurysm risk factors? |
Associated with tobacco smoking, HTN, family history (i.e. atherosclerosis) |
Define Intermittent Claudication
Intermittent reproducible, burning ischaemic pain that presents with activity (walking) a specific distance and is relieved by rest. Usually in the calves, narrowing is the level above (ie calves --> SFA, thighs --> Iliac)
Causes of acute limb ischaemia? and clinical differences?
- Acute thrombosis 60% (pre-existing atherosclerosis) Clinically - prev hx of I/M claudication, slow onset, no obvious embolic source, reduced/absent pulses in contralateral limb.
- Emboli (30%) - 80% cardiac source, Clinically - No Hx Claudication, Rapid onset complete occlusion, cardiac dysrhythmias. - Other - Aortic dissection, trauma, iatrogenic |
Clinical features of acute limb ischaemia?
6 P's Pain Pallor Pulselessness Paraesthesia Paralysis Perishingly cold (poikilothermia) |
