What are the requirements for using an alternate method other then the LP blower to de-water a MBT?
- Safety of Ship Officer (SOSO) and SDO must be notified
- The air source must be identified to the ships force. A ships “CAUTION” tag shall be posted on the air supply valve with special instruction stating “THIS VALVE IS TO BE USED AS AN ALTERNATE MEANS TO DEWATER THE MAIN BALLAST TANK __.”

What is the function of the RCL?
- The RCL releases the work in the TGI. It will indicate if a WAF is required, and what steps in the TGI you are allowed to complete.

What do you do if one of your divers cannot accomplish a step in the TGI?
- Have the diver stop work. Seek clarification from the lead trade and/or engineering. If step cannot be completed submit DL.
What type of work can be accomplished under a dive safety WAF?
Most inspections and hull surveys. The required RCL will indicate if a WAF is required or not.
Who can authorize the modification of a tag out?
- The diving supervisor with concurrence from the SDO/CDO is authorized to modify a standard ships tag out.

The diving supervisor or designated representative is required to brief with the CDO/SDO.

C/246, Dive Team, Ship Superintendent, Ship’s Force and lead trade.
What is a PRL, and when is it required?
Pre Requisite List and it is required for all CNO and FMR work.
A dive TGI lists the minimum anti c’s for a contamination dive as short sleeve

What is your course of action?
- Stop work and submit a DL. All contamination dives require a full length wetsuit, booties, hood and rubber gloves with arm sleeves or gauntlet style rubber gloves.
The lead trade and ships force ask if your divers could witness the raising and lowering of the SPM on a SPM inspection TGI. What do you tell them?
- Ensure the proper Industrial Process Instruction (IPI) is referenced in the TGI for a Potentially Dangerous Evolution (PDE). If it is not then a DL needs to be submitted to include the proper IPI.
During a ballasting evolution for a cable pass through from MBT 4 A/B or 5 A/B to the surface the diver reports the hull penetrator is not underwater. What is your course of action?
- Instruct the diver to not dissemble the penetrator. Immediately notify the SDO and the SOSO for further guidance.
During a ballasting evolution for a cable pass through from the upper level of the forward MBT’s, 1 A/B 2 A/B or 3A/B, the diver reports the hull penetrator is not underwater. What is your course of action?
- Ensure the MBT is vented to atmospheric pressure. This can be accomplished by having the ships force open a vent path to surface through the bridal blow or taking the depth on the diver in the MBT using the pneumofathometer.
A civilian gets a deep cut on his hand from a piece of metal while tending. What are you as a supervisor required to do?
- Ensure employee is OK and no one else can be injured. Fill out a dispensary permit and send him to the shipyard dispensary.
Your diver is conducting a MBT close out inspection after finishing an ABT plug removal. He reports there is a grease line that has come undone from MBT 3 vent valve and would like to re-connect it. What is your course of action?
- Tell him not to re-connect the grease line. Assist the mechanic in submitting a DL/DR, it is an existing condition outside the scope of the TGI.
Before entering a MBT how do you verify 2 valve protection?
- Ask the Ships Duty Officer to show on the SOMS the valves red tagged for two valve protection. These valves can be verified using the EMBT piping diagram. If there are any questions seek clarification from C/246.

During hull inspections or swipes and growth on a submarine how is the sub to tag its suctions?
- The ASW has to be in slow with fast speed pushbutton tagged out and/or the MSW has to be in super slow with the fast speed breaker tagged out. The ASW push button operator is often not on the tag out. The dive sup has to check and make sure the tags were added
25) What is the course of action when an unexpected or unplanned event happens during crane operations? I.e. dropped load, overload, damage to the load or equipment, injury to personnel

Assist the riggers/crane operator in putting the crane and load in a safe condition.

- Provide ABC’s and BCLS as required and immediately transport to the duty chamber to be evaluated by a DMO.

The OSHA requirement for commercial diving operations that civilians are required follow.
What is the requirement for civilian diver carried reserve requirement when diving SCUBA?
A manual J valve or an independent reserve cylinder with the valve in the closed position.

It gives the gives the distance to any hull opening or appendage in feet and inches in relation to aftermost point on the ship or sub.
Were can you find guidance on the removing UBA in a submarine ballast tank and what are the requirements the submarine must be aware of prior to the dive?
- The dive manual chapter 8-11.4.2 gives guidance on removing the UBA. PHNSY & IMF follow NAVSEA S-6470-AA-SAF-010 (known as the SAF-10) for gas free requirements for a shore based facility. The divers shall not remove their diving equipment until the atmosphere has been flushed twice with air from a compressed air source meeting the requirements of chapter 4, or submarine LP blower.
What is the course of action when an employee receives an electrical shock?
Provide medical assistance as required and take steps to prevent anyone else form being shocked. Anyone who has received an electrical shock must be evaluated by medical personnel. If the shock is low voltage with no visible entry or exit wounds evaluation by an ambulance EMT is adequate. If transport is necessary they cannot be driven there by co-workers or supervisor and ambulance must provide transport.
What steps do you take if an “undesirable event” happens during work, i.e. Wrong MBT flooded, valve in wrong position, unplanned cycling of ships equipment?
- Stop the evolution in progress. Direct divers/ships force to put the system in a safe condition.
