Testicular Torsion
|
Spermatic cord torsion occur with sudden onset of severe pain. N/V maybe present. It's maybe preceding trauma.
Assess patient in upright position. Elevate involved testicle, exacerbate non-torsion pain, this would produce relief due torsion. Surgical consult required. |

Epididymitis
|
Often begins with arthritis, than inguinal soreness, than very tender epididymis.
Treat with antibiotic. |

Orchitis
|
Follow systemic viral infection, particularly mumps, after 7-10 days, mostly unilateral swelling, discomfort.
Increase amylase associated with mumps. Testicular tumor: painless mass. Pt may complaint of heaviness sometimes with pain in hemorrhage into malignancy occurs. Increase with hx of Undescended testicle. Monitor young man (15-35 years) monthly as a screening recommendation. Hydrocele: soft mass, no age predilection. Positive trans-illumination. R/O hernia. Ultrasound required. Varicosele: "bag of warm", sometimes associated with infertility. More common left side. Usually nontender, chronic, regresses in supine position. Banalitis: gland infection. Most often in uncircumcised man. Better hygiene is the treatment. Steroid cream to decrease inflammation, anti fungal or antibacterial cream can also be used. Phimosis: difficulty with foreskin retraction. Paraphimosis: inability to retract foreskin. Urology consult immediately. |
