Dr. Roy W. Sweat
When was the instrument first created?
1970
Who came before AO and Dr. Sweat?
BJ Palmer and John F. Grostic
Why take post films?
To see how the patient responded structurally to the adjustment,
To help us better manage the patient's case, To help guide us in changing the instrument setting if necessary |
Definition of "orthogonal"
straight angle,
the head and neck are at right angles (perpendicular) to the atlas, and the atlas is level |
Basic principle: What is one of the major purposes of the spine?
to support the weight of the head
What happens when the weight of the head becomes off center or leaves vertical?
the lower areas begin to compensate to maintain the center of gravity
Where are the center of the head and shoulders always maintained?
over the center of the feet
Many LLI correct with
C1 adjustment
Mission statement
To provide excellent compassionate healthcare services to all patients,
To care for the needs of all people with the highest standards of care possible, To promote better health in the world and in our communities through involvement, education, and wellness, To be efficient and cost effective |
As it passes through the atlas transverse foramen and continues up into the head, how many 90 degree turns does the vertebral artery take?
4
Dr. Roy W. Sweat has not adjusted any vertebra below the atlas since
1952
In the Atlas Orthogonal adjusting program, we believe that adjusting the atlas and upper cervical area is not a simple procedure. It is as delicate as
the most complicated brain surgery
How many views are taken "pre" adjustment in AO?
5
What are the AO "pre" views?
Sagittal, Frontal, Horizontal, AP open mouth, AP cervical
What are the standard AO "post" views?
Horizontal and Frontal
How do we know at what angle the central ray should be for the frontal film?
from the Sagittal ASP line
How do we tilt the bucky for the sagittal view?
Head tilt to remove appearance of head tilt on film
How do we tilt the bucky for the frontal view?
none
How do we tilt the bucky for the Horizontal view?
30 degrees
What do we do if there is a wisdom tooth visible on the sagittal film, and the ASP line goes through it?
shoot at higher ASP
Do we ever move the head to take an xray?
No
Placement of hard palate for all AO views
level with the floor
How do we remove head tilt for the horizontal film?
tilt at waist
A filter is used when we take the ? , for better visualization of the structures
horizontal view
Head clamps
used on all AO xray views
What do we outline on the horizontal film?
lateral masses
What is the shape of the lateral mass on the horizontal film?
peanut
Numer of lines on sagittal film
1
Number of lines on frontal film
3
Number of lines on horizontal film
2
Which film has the ASP (atlas sagittal plane) line?
SAGITTAL
Which film has the AFP (atlas frontal plane) line?
Frontal
What is the ACD?
frontal - angle between center skull ine and atlas plane line, upper angle, = atlas cephalic displacement
What is CS?
cervical spine, lower angle
What does ipsilateral mean?
same side
What does contralateral mean?
opposite sides
What are the factors we look at for determining typical high or typical low?
ACD and CS: Contralateral or ipsilateral?,
How high is the AFP line?, Side of axis spinous rotation?, Atlas anterior or posterior rotation on the ACD side? |
Typical high line drawing from frontal x-ray film
ACD and CS contralateral,
SP ipsilateral to ACD, ACD high on right, Atlas anterior right |
Typical low line drawing from frontal x-ray film
ACD acute on right,
ACD and CS ipsilateral, SP rotated opposite ACD, +/- 2 mm APL typically lower, Atlas posterior Right |
How are the ACD and CS related for typical high listings?
contralateral
How are the ACD and CS related for typical low listings?
ipsilateral
Where has the spinous of C2 rotated in a typical high listing?
same side as the ACD
Purpose of the horizontal film?
atlas rotation
Where on the patient is the scanning palpation done?
Over C1 and C2 dorsal nerve root ganglia bilaterally
What is the inter-rater accuracy of the AO leg check?
Within 1/8 of an inch
When do the leg check?
pre & post adjustment
Should the patient rest after the adjustment? How long?
Supine 3-5 min, adjustment sets, see change in leg length
Do we touch the legs during the leg check measurement?
No, just place legs, no take out eversion/inversion, just make sure legs are centered
What is the distance between lines on the leg check grid?
1/4"
When is scanning palpation done?
both before and after the adjustment
What does it mean that 50% of the adjustment is what happens above, and 50% of the adjustment is what happens below?
patient positioning below, instrument above
The patient's head is placed on the headpiece so that it is supported at the
mastoid process
The AO adjustment is delivered to the
transverse process of atlas
How far inside skin is the transverse process of atlas
1/2 - 1 inch
When we laterally flex the neck to the right, the C2 spinous goes
left
What do pre- and post- leg length measurements tell us?
We are adjusting on the correct side,
If we need to change the instrument setting for the rotational vector |
What pre-empts the leg check?
Scanning palpation
Dr. Blair's history
suffered from severe asthma is entire life. Medical treatment didn’t help and he was told he would not be able to live much past his early 20s. He was even referred to as a “one time simi-invalid”. His asthma had grown so severe, he was told to move to a more favorable climate. While in El Paso where he met and married Beatrice "Dotty" Kobold on July 20, 1946. Bill was introduced to upper cervical chiropractic by his wife Dotty. Her mother had been a life long friend of a Chiropractor located in Houston.
Reluctantly, due to previous chiropractic shortcomings, he was persuaded to become a patient of Dr. Leon Halsted. He received his first Upper Cervical Chiropractic adjustment at this time. After only a few months of care he noticed a miraculous change in the improvement of his asthmatic symptoms. So much so, that he and his wife packed up their belongings and moved to Davenport, Iowa where he entered Palmer College of Chiropractic.graduated in 1949 |
