5 Cards in this Set
- Front
- Back
An acute, rapidly progressing and potentially fatal form of polyneuritis
-affects the peripheral nervous sytem and results in loss of myelin and edema and inflammation of the affected nerves, causing loss of neurotransmission to the periphery |
-Guillian-Barre Syndrome (GB)
Describe the etiology of GB:
-affects more males than females
-typically seen in adults though can occur at any age -85 to 95% of patients recover completely -cause is unknow though thought to be a cell-mediated immunologic reaction directed at the peripheral nerves -often preceeded by immune system stimulation from a viral infection (Campylobacter jejuni), trauma, surgery, viral immunizations, or HIV -stimuli are thought to cause an alteration in the immune system (T and B cells) resulting in myelin damage -B/c this damage, nerve impulses are stopped or slowed down -in the recovery phase, remyelination occurs slowly and neurologic function returns in a proximal-to-distal pattern |
Describe signs and symptoms for a patient with GB:
-range from mild to severe
-usually develops 1-3 weeks after an upper resp or GI infection -weakness of the lower extremities, usually peaking about the fourteenth day -paresthesia -areflexia -hyptonia -ANS: resp muscle paralysis. orthostatic hypotension, HTN, abnormal vagal responses (heart block, bradycardia, asystole), bladder and bowel dysfunction, facial flushing, and diaphoresis -Can involve the face causing paralysis -Pain is common (opiods if severe) |
What is the most serious complication of GB?
Respiratory failure
**check RR and depth, forced VC, and IS **other complications include: URI and UTI (fever is 1st sign), paralytic ileus, muscle atrophy, DVT, PE, skin breakdown, orthostatic hypotention, nutritional deficiencies |
Describe nursing assessements for the GB patient:
-MOST IMPORTANT ASPECT OF NURSING WITH A GB PT
-Monitor: ascending paralysis; assess resp function, ABGs, gag, corneal, and swallowing reflexes, BP, HR and rhythm -meticulous suctioning -intermittent cath -ROM |
