Why do exo pts need treatment?
they need much effort to keep their eyes straight - therefore asthenopic symptoms are important
even with good control, X's and X(T)s can still be symptomatic; but also may be just observation as treatent
Your X(T) patient only loses control at near after being fully dissociated, and quickly regains fusion & is ortho again without re-fixating. what is your treatment? Your patient is 6 years old.
Observe - since the pt is visually immature and has excellent control of the X(T); also this pt always has BSV

good - no spontaneous break, breaks with CT and quickly regains fusion without blinking or refixating

fair - no spontaneous break, but breaks on CT and has to re-fixate or blink to regain fusion

poor - breaks spontaneously in the office without disruption iof fusion
Describe some tests you can do in clinic to assess control of pt's XT
- near and distance monocular acuity
- jump convergence
- NPC target/red filter
- BVA
- measure to light
- +3.00s
- CT/occlusion test
- recovery (good, bad, fair)
how are spectacles an aid in controlling exodeviation
better acuity = better stimulus to fuse especially witrh astigmatism and amblyopia
ALSO, minus lenses (myopic correction) stimulates accommodation and therefore accommodative convergence, and can help with exo angle
how can prism therapy be used for exos?
BI prism - fuse pre-op or alleviate asthenopia/diplopia

*use least amount of prism needed, and try to reduce each visit *
your patient is 7 yo and has X(T) 20 pd, and is symptomatic. can you do orthopic treatmentr?
yes, since dev < 25pd, if larger usually Sx
