42 days
Storage temperature for whole blood or pRBC?
1-6 C
Shelf life for frozen RBC? What is the storage temperature? What is the shelf life after thawing?
1. Frozen RBC: 10 years
2. Storage temperature: -65 C
3. Shelf life after thawing: 24 h
Shelf life for washing RBC? Storage temperature?
24 h at 1-6 C
Shelf life of platelets? Storage temperature?
5 days at 20-24 C with gentle agitation
Shelf life of granulocyte transfusion? Storage temperature?
24 h at 20-24 C without agitation
Shelf life of FFP and storage temperature? (3)
1. 1 year at -18 C
2. 7 years at -65 C
3. 24 h at 1-6 C (after thaw)
Shelf life of cryo and storage temperature? (3)
1. 1 year at -18 C
2. 6 hours at 20-24 C (after thaw single donor)
3. 4 hours at 20-24 C (pooled products)
What is the maximum hematocrit for RBC?
Less than 80%
What is the maximum WBC count for leukoreduced RBC or leukoreduced apheresis platelets?
Less than 5 x 10^6
What is the minimum platelet count for 1 unit of platelet concentrate?
At least 5.5 x 10^10
What is the maximum WBC count for leukoreduced platelet concentrate? (Clue: it is 1/6 of that for leukoreduced RBC or apheresis platelets.)
Less than 8.3 x 10^5
What is the minimum platelet count for apheresis platelets?
At least 3 x 10^11
What is the minimal WBC count for granulocyte infusion?
1 x 10^10
What are the components of cryo and what are the minimal concentrations?
Factor 2 (fibrinogen): at least 150 mg
Factor 8 at least: 80 IU
VWF: 80-120 IU
Factor 13: 40-60 IU
What is the expected bump in hemoglobin and hematocrit for 1 unit of RBC?
HB = 1
Hct = 3
What is the typical volume for the following products?
RBC
Platelet concentrate platelets
Apheresis platelets
FFP
Cryo
RBC: 250 mL (350 mL with additives)
Platelet concentrate platelets: 40-60 mL
Apheresis platelets: 100 mL
FFP: 200-250 mL
Cryo: 15 mL
How do you dose platelet concentrate platelets in adults in babies?
Adults: 1 unit per 10 kg body weight
Babies: 10-15 mL/Kg
Is leukoreduction sufficient for people at risk for GVHD?
No, these need to be irradiated.
What is the shelf life of washed RBC? What about platelets?
RBC: 24 h
Plt: 4 h
What is the shelf life of thawed/deglycerolized frozen RBC? What temperature should it be stored at?
24 h at 1-6 C.
What is the clinical presentation of TA-GVHD?
1. Fever 7-10 days after transfusion
2. face/trunk rash that spreads to extremities
3. Diffuse mucositis with N/V, watery diarrhea
4. Hepatitis
5. Pancytopenia, aplastic anemia
What dosage of radiation is given for irradiated products?
1. 2500 cGy ("rad") at center of bag and
2. 1500 cGy in all parts of bag
Which immunocompetent recipient needs irradiated products?
HLA "matched" unit.

NOTE: E.g. close relatives - if recipient is heterozygous and donor is homozygous for and they share an allele, the recipient will not be mount a response to the donor lymphocytes.
What is a product's shelf life after irradiation?
28 days after irradiation or regular expiration date, whichever comes first.
What do these people have in common:
Organ transplant recipients
Term neonates
Aplastic anemia
AIDS
They don't have much risk for GVHD but often get irradiated products anyway.
What is "thawed plasma"?
Thawed FFP is only good for 24 h. After 24 h, the product can be relabeled as "thawed plasma" and kept at 1-6 C for up to 5 days
What is "liquid plasma"?
"liquid plasma" is separated from whole blood at any point up to 5 days after expiration, stored at 1-6 C for up to 5 days AFTER whole blood expiration date.
Is irradiation of granulocyte transfusion allowable?
Yes, may irradiate granulocyte transfusion to prevent GVHD because the radiation kills lymphocytes but not granulocytes for some reason.
What is the shelf life of granulocyte transfusion? Stored at what temperature?
24 h at 20-24 C
Do you need to type and crossmatch granulocyte transfusions?
Yes, there are a lot of RBC in there.
