Hydrocodone w/ APAP/Vicodin
- Narcotic Analgesic & antitussive
- Opiod derived from codeine & thebaine
- Psychological & physical addiction
- Dispensed 5-10mg w/ 500 mg of acetaminophen
- DEA schedule III (5-10 mg)
- DEA schedule II ( >15 mg)
- SE: Respiratory depression; N/V; constipation
- MOA: Binds to mu receptors in brain and spinal cord reducing pt perception of pn.

Acetaminophen
- Found in cerebral cortex & heart = COX 3
- COX 3: reduces pn and fever w/o reduction in inflamm
- Metabolized by PHASE II metabolism; utilizes sulfate & glucuronide
- Cyt P450 2E1 & P450 3A4 convert to NAPQI (n acetyl p benzo quinone imine) which has high rate of hepatic toxicity; rqrs glutathione
- Decreased lvls of glutithione may precipitate acute hepatic necrosis
