Thioamides

MOA: Inhibits oxidation of iodide, iodination of tyrosine and coupling.




Propylthiouracil/Methimazole/Carbimazole




Tx Graves Disease



Don't use Methimazole during pregnancy


Methimazole more potent and cross placenta.


ADR: Rash, fever, GI intolerance, joint pain

Iodine/Iodide

MOA: Inhibits proteolysis of Tgb in thyroid follicle and release into bloodstream




Tx: Surgical Prophylaxis, Thyroid storm, radio contrast media

Beta Blocker/Corticosteroids

MOA: Prevent iodine from being removed from Tgb.




Propanolol


Prednisone (treat exophthalmos)




Tx: Symptomatic relief, Thyroid Storm

Radioactive Iodine

MOA: Emit radiation to destroy thyroid tissue




I 131




Tx: Thyroid storm, Pre-surgical treatment of thyroid, radio contrast

Slow acting


ADR: Hypothyroidism requiring replacement therapy.


Incr risk of leukemia


Not to be used on females of child bearing age

T4

Tx; Creatinism, Adult Hypothyroidism, Myxoedema coma, Nontoxic Goitre

T3

Tx: Myxodema Coma
